彩色多普勒超声对乳腺癌及非哺乳期乳腺炎的诊断鉴别

Color Doppler ultrasound in the diagnosis and differentiation of breast cancer and non-lactating mastitis

目的 探讨彩色多普勒超声对乳腺癌及非哺乳期乳腺炎鉴别诊断作用。方法 选取我院2022年2月至2023年2月收治的42例乳腺癌患者为A组,另选同期我院收治的43例非哺乳期乳腺炎患者为B组,所有患者均行彩色多普勒超声检查,比较2组诊断结果、超声图像差异,评价超声对2种疾病的鉴别诊断作用。结果 以病理结果为金标准,彩色多普勒超声对乳腺癌总体诊断符合率为88%,对非哺乳期乳腺炎总体诊断符合率为86%,均与病理结果差异无统计学意义(P>0.05);乳腺癌患者超声图像特征主要以形态欠规则、边界不清晰、病灶后回声无衰减、不存在囊性成分、有微小钙化、腋窝淋巴结内部结构紊乱、Ⅱ~Ⅲ级血流信号、阻力指数(RI)≥0.7为主,非哺乳期乳腺炎患者则相反,2组超声图像特征相比差异有统计学意义(P<0.05)。结论 彩色多普勒超声在乳腺癌、非哺乳期乳腺炎中诊断价值较高,可有效对两者进行鉴别诊断,为临床制定后续治疗方案提供有力证据,值得应用。

Objective To explore the role of color Doppler ultrasound in the differential diagnosis of breast cancer and non-lactating Mastitis.Methods A total of 42 patients with breast cancer admitted to our hospital from February 2022 to February 2023 were selected as group A,and 43 patients with non-lactating Mastitis admited to our hospital in the same period were selected as group B.All patients were examined by color Doppler ultrasound,and the diagnostic results and ultrasound image diferences between the two groups were compared to evaluate the role of ul-trasound in the differential diagnosis of the two diseases.Results With the pathological results as the gold standard,the overall diagnostic accuracy of color ultrasound for breast cancer was 88%,and for non-lactating mastitis was 86%.There was no significant difference between them based on pathological results(P>0.05).The ultrasonic image features of breast cancer patients mainly included irregular shape,unclear boundary,no attenuation of echo after the focus,no cystic component,microcalcification,disorder of internal structure of axillary lymph nodes,grade II~II blood flow signals,RI≥0.7,while the ultrasonic image features of non-lactating Mastitis patients were on the contrary,and there was a significant difference between the two groups(P<0.05).Conclusion Color Doppler ultrasound has high diag-nostic value in breast cancer and non-lactating Mastitis.It can effectively differentiate the two and provide strong evi-dence for the clinical development of follow-up treatment plans,which is worthy of application.
