摘要
目的探讨血清微小RNA-485(miR-485)和miR-30的表达对糖尿病肾病(DN)患者的诊断价值。方法选取2022年1月至2023年1月我院收治的96例DN患者为DN组,同时期本院收治的96例单纯糖尿病患者为糖尿病组,同时期在本院进行体检的96例健康志愿者为对照组。采用实时荧光定量聚合酶链式反应(qRT-PCR)测定3组人员血清miR-485、miR-30表达水平;采用Logistic模型分析DN发生的影响因素,并采用受试者工作特征(ROC)曲线评价血清miR-485、miR-30水平对DN的诊断价值。结果对照组、糖尿病组、DN组血清miR-485与miR-30表达水平均依次降低(P<0.05)。DN组患者糖尿病病程、空腹血糖、餐后2 h血糖、血肌酐、胆固醇、低密度脂蛋白胆固醇水平均高于糖尿病组,高密度脂蛋白胆固醇水平低于糖尿病组,差异均有统计学意义(P<0.05)。糖尿病病程、餐后2 h血糖、血肌酐、胆固醇、低密度脂蛋白胆固醇均是导致DN发生的危险因素(P<0.05),高密度脂蛋白胆固醇、miR-485、miR-30表达水平均是保护因素(P<0.05)。血清miR-485、miR-30联合诊断DN的ROC曲线下面积(AUC)为0.914,高于二者单独诊断的AUC(Z=2.168,P=0.030;Z=2.882,P=0.004)。结论DN患者血清miR-485、miR-30表达均下调,二者均是DN发生的保护因素,可作为诊断DN的临床指标,且二者联合诊断的效果更好。
Objective To investigate the diagnostic value of serum microRNA-485(miR-485)and miR-30 expression in patients with diabetic nephropathy(DN).Methods A total of 96 patients with DN admitted to our hospital from January 2022 to January 2023 were regarded as the DN group,96 patients with simple diabetes admitted to our hospital were collected as the diabetes group,and 96 healthy volunteers who underwent physical examination in our hospital were regarded as the reference group.Real time fluorescence quantitative polymerase chain reaction(qRT-PCR)was applied to determine the serum miR-485,miR-30 expression levels of the three groups;Logistic model was applied to analyze the influencing factors of DN occurrence;Receiver operating characteristic(ROC)curve was applied to evaluate the diagnostic value of serum miR-485 and miR-30 levels for DN.Results The expression levels of serum miR-485 and miR-30 in the reference group,diabetes group and DN group decreased in turn(P<0.05).The course of diabetes,2 h postprandial blood glucose,serum creatinine,cholesterol,and low-density lipoprotein cholesterol in DN group were higher than those in diabetes group,and the level of high-density lipoprotein cholesterol was lower than that in diabetes group(P<0.05).The course of diabetes,2 h postprandial blood glucose,serum creatinine,cholesterol,and low-density lipoprotein cholesterol were all risk factors for DN(P<0.05),high-density lipoprotein cholesterol,miR-485,and miR-30 expression levels were protective factors(P<0.05).The area under the receiver operating characteristic(AUC)of serum miR-485 and miR-30 in the diagnosis of DN was 0.914,which was higher than the AUC of serum miR-485 and miR-30 alone(Z=2.168,P=0.030;Z=2.882,P=0.004).Conclusion The expression levels of miR-485 and miR-30 in the serum of DN patients are down regulated,and both are protective factors for the occurrence of DN.They can be used as clinical indicators for the diagnosis of DN,and the combined diagnosis of the two is more effective.
