冠心病患者PCI术后心血管事件发生的影响因素分析

Influence factors for cardiovascular event occurrence in patients with coronary heart disease after percutaneous coronary intervention

目的探讨冠状动脉粥样硬化性心脏病(冠心病)患者经皮冠状动脉介入治疗(PCI)术后心血管事件发生的影响因素。方法选择2021年1月至2023年1月于青岛市市立医院治疗的冠心病患者121例进行研究,均行PCI。收集患者临床资料,分析PCI术后心血管事件的危险因素。结果本研究的121例患者均顺利完成PCI,手术时间45~60 min,平均时间为(53.25±3.14)min。术后随访发现,25例发生心血管事件,包括再发心绞痛、心肌梗死、心力衰竭和心律失常。单因素分析显示,两组的年龄、高血压、高血脂、糖尿病、NYHA心功能分级、吸烟、饮酒及冠状动脉病变支数比较,差异有统计学意义(P<0.05)。多因素Logistic分析显示,年龄、吸烟、高血压、高血脂、糖尿病、NYHA心功能分级、饮酒及冠状动脉病变支数均是冠心病患者PCI术后发生心血管事件的独立危险因素(P<0.05)。结论冠心病患者PCI术后发生心血管事件的独立危险因素较多,临床应重点关注。

Objective To investigate the influence factors for cardiovascular event occurrence in patients with coronary heart disease(CHD)after percutaneous coronary intervention(PCI).Methods CHD patients undergone PCI(n=121)were chosen from Qingdao Municipal Hospital from Jan.2021 to Jan.2023.The clinical materials were collected in the patients,and risk factors for cardiovascular event occurrence after PCI were analyzed.Results All 121 patients completed PCIsuccessfully,and operation time was from 45 min to 60 min and average time was(53.25±3.14)min.The results of postoperative follow-up showed that cardiovascular events occurred in 25 patients,including relapsed angina pectoris,myocardial infarction,heart failure and arrhythmia.The results of single-factor analysis showed that the differences in age,hypertension,hyperlipidemia,diabetes,NYHA classification of heart function,smoking,drinking and number of diseases coronary vessels had statistical significance between 2 groups(P<0.05).The results of multi-factor Logistic analysis showed that age,smoking,hypertension,hyperlipidemia,diabetes,NYHA classification of heart function,drinking and number of diseases coronary vessels were independent risk factors for cardiovascular event occurrence after PCI.Conclusion There are more independent risk factors for cardiovascular event occurrence in CHD patients after PCI,which should be paidclose attention to in clinical practice.
