经脐单孔法行腹腔镜阑尾切除术和开腹阑尾切除术治疗阑尾炎的临床疗效对比研究

Comparative Study on the Curative Effect of Transumbilical Single-port Laparoscopic Appendectomy and Open Appendectomy Through Umbilical Single Hole in the Treatment of Appendicitis

目的 分析经脐单孔法行腹腔镜阑尾切除术、开腹阑尾切除术治疗阑尾炎的临床效果。方法 选取2022年1月—2023年12月永春县中医院普外科收治的152例阑尾炎患者为研究对象,以不同手术方式分为对照组和观察组,各76例,对照组采用开腹阑尾切除术,观察组采用经脐单孔法行腹腔镜阑尾切除术。分析两组的围手术期指标、术后并发症和炎性因子水平。结果 观察组的手术时间、术后下床时间、术后首次排气时间、肠鸣音恢复时间、进食恢复时间、住院时间短于对照组,术中出血量少于对照组,差异有统计学意义(P均<0.05)。术后,两组患者的炎性因子水平均降低,且观察组低于对照组,差异有统计学意义(P<0.05)。观察组术后并发症发生率为1.32%(1/76),低于对照组的10.53%(8/76),差异有统计学意义(χ^(2)=4.252,P<0.05)。结论 阑尾炎患者采用经脐单孔法行腹腔镜阑尾切除术效果理想,可加速患者术后恢复进程,在降低并发症发生风险的同时有效减轻机体炎性因子水平,具有较高的临床价值。

Objective To analyze the clinical effect of transumbilical single-port laparoscopic appendectomy and open appendectomy in the treatment of appendicitis.Methods A total of 152 patients with appendicitis admitted to the De⁃partment of General Surgery,Yongchun County Traditional Chinese Medicine Hospital from January 2022 to Decem⁃ber 2023 were selected as the research objects.They were divided into control group and observation group according to different surgical methods,with 76 cases in each group.The control group was treated with open appendectomy,and the observation group was treated with transumbilical single-port laparoscopic appendectomy.The perioperative indi⁃cators,postoperative complications and inflammatory factor levels of the two groups were analyzed.Results The opera⁃tion time,postoperative ambulation time,postoperative first exhaust time,bowel sound recovery time,eating recovery time and hospitalization time of the observation group were shorter than those of the control group,and the intraopera⁃tive blood loss was less than that of the control group,the differences were statistically significant(all P<0.05).After operation,the levels of inflammatory factors in the two groups decreased,and those in the observation group were lower than the control group,the differences were statistically significant(P<0.05).The incidence of postoperative complications in the observation group was 1.32%(1/76),which was lower than 10.53%(8/76)in the control group,and the difference was statistically significant(χ^(2)=4.252,P<0.05).Conclusion The effect of transumbilical single-port laparoscopic appendectomy in patients with appendicitis is ideal,which can accelerate the recovery process of patients after operation,reduce the risk of complications and effectively reduce the level of inflammatory factors in the body,and has high clinical value.
