解毒化瘀汤联合曲安奈德口腔软膏治疗口腔扁平苔藓30例

Treatment of Oral Lichen Planus with Detoxification and Stagnation-resolving Decoction Combined with Triamcinolone Acetonide Dental Paste:30 Cases

目的:观察曲安奈德口腔软膏联合解毒化瘀汤治疗口腔扁平苔藓的临床疗效。方法:回顾性选取口腔扁平苔藓患者60例,依据不同用药方法分为观察组与对照组,每组30例。观察组采用曲安奈德口腔软膏联合解毒化瘀汤治疗,对照组采用曲安奈德口腔软膏治疗。观察两组临床疗效、口腔疼痛程度、口腔黏膜病损面积、症状评分、炎症因子、免疫因子水平及复发情况。结果:观察组总有效率[96.67%(29/30)]高于对照组[73.33%(22/30)](P<0.05);治疗后观察组数字分级评分法评分、口腔黏膜病损面积均小于对照组(P<0.05);治疗后观察组症状评分、白细胞介素6(interleukin,IL-6)、IL-8、IL-10、肿瘤坏死因子α(tumor necrosis factor,TNF-α)水平及CD8+均低于对照组(P<0.05),CD4+、CD4+/CD8+均高于对照组(P<0.05);观察组复发率[23.33%(7/30)]低于对照组[53.33%(16/30)](P<0.05)。结论:曲安奈德口腔软膏联合解毒化瘀汤治疗口腔扁平苔藓,可缩小口腔黏膜病损面积,降低炎性指标水平,提高临床疗效,降低复发率。

Objective:To observe clinical effects of triamcinolone acetonide dental paste and detoxification and stagnation-resolving decoction in the treatment of oral lichen planus.Methods:Sixty patients were retro-spectively chosen and divided into the observation group and the control group according to the medication methods,30 cases in each group,the observation group was treated with triamcinolone acetonide dental paste and detoxification and stagnation-resolving decoction,and the control group accepted triamcinolone acetonide dental paste.To observe clinical efects,oral pain degrees,the area of oral mucosa lesion,symptom scores,the levels of inflammatory factor and immune factor,and the recurrence conditions between the two groups.Results:Total effe-ctive rate of the observation group was[96.67%(29/30)],higher than[73.33%(22/30)]of the control group(P<0.05);after the treatment,the observation group was less than the control group in numerical rating scale(NRS)scores and the lesion area of oral mucosa(P<0.05);after the treatment,symptom scores,the levels of IL-6,IL-8,IL-10 and TNF-α,as well as CD8+of the observation group were lower than these of the control group(P<0.05),the levels of CD4+,CD4+/CD8+were higher than these of the control group(P<0.05).The relapse rate of the observation group was[23.33%(7/30)],lower than[53.33%(16/30)]of the control group(P<0.05).Conclusion:Triamcinolone acetonide dental paste and detoxification and stagnation-resolving decoction in the treatment of oral lichen planus could reduce the lesion area of oral mucosa,lower the levels of inflammatory factors,and raise curative effects,as well as decrease the recurrence rate.
