诊断或处理失误致阑尾切除术后再手术12例分析

Analysis of 12 reoperation cases after appendectomy resulted from the misdiagnosis or inappropriate surgical treatment.

目的 探讨由于诊断或处理失误致阑尾切除术后再手术的原因。方法 总结分析 1991～ 2 0 0 0年因诊断或处理失误导致阑尾切除术后再手术的病例。结果 因诊断或处理失误导致阑尾切除术后再手术 12例 ,包括回盲部肿瘤漏诊 4例 ,阑尾残株过长和遗留远侧阑尾各 1例 ,阑尾系膜出血 2例 ,大网膜粘连综合征 2例 ,盲肠扭转 1例 ,宫外孕误诊 1例 ,再次手术后全部治愈。结论 诊断或处理失误 ,缺乏经验 ,对阑尾炎和阑尾切除术重视不够 。

Objective To analyze the causes of reoperation after appendectomy resulted from misdiagnosis and inappropriate surgical treatment. Methods The patients who had undergone reoperation after appendectomy because of the misdiagnosis or inappropriate surgical treatment in our hospital were retrospectively reviewed from 1991 to 2000.Results There were 12 cases of reoperation after appendectomy because of the misdiagnosis or inappropriate surgical treatment. These included cecum tumor in 4 cases,stump appendicitis 1 case,residual distal appendicitis 1 case,bleeding of mesoappendix 2 cases,omental adhesion syndrome 2 cases,volvulus of cecum 1 case and ectopic pregnancy 1 case.All of the patients recovered smoothly after successful reoperation.Conclusion The misdiagnosis or inappropriate surgical treatment,lacking of experience and underestimating appendectomy are the main causes of reoperation after appendectomy.
