摘要
目的 研究肾素 -血管紧张素系统 (RAS)中的Ⅰ型血管紧张素Ⅱ受体 (AT1R)基因A116 6C多态及血管紧张素Ⅰ转化酶 (ACE)基因插入 /缺失 (I/D)多态与中国汉族 2型糖尿病 (DM 2 )患者合并冠心病(CHD)的关系。方法 分别用PCR技术及PCR -Dde1酶切法检测 15 2例DM 2患者的ACE基因型及AT1R基因型。其中DM2合并CHD组 6 6例 ,无CHD组 86例。结果 DM2 -CHD组与无CHD组相比 ,AT1R基因型频率及等位基因频率均无显著性差异 (P >0 .0 5 ) ;DM 2 -CHD组ACEDD基因型频率 (5 1.5 %vs32 .2 % )和D型等位基因频率 (6 8.2 %vs 5 6 .3% )均显著升高 (P均 <0 .0 5 )。Logistic回归分析显示 :ACEDD基因型、BMI及血清总胆固醇水平升高是DM 2 -CHD发病的独立性危险因素 (P <0 .0 5 )。结论 AT1R基因A116 6C多态不参与DM2 -CHD的发病 ;ACE基因I/D多态与DM2 -CHD有关 ,在DM2患者中检测ACE基因I/D多态 ,可筛选CHD易感人群 ,进行发病前诊断和治疗。
Objective:To explore the association between gene polymorphism of Renin-angiotensin system (RAS) and aged type Ⅱ Diabetes (DM2) complicated with coronary heart disease(CHD).Methods:ACE genotypes and AT1R genotypes were investigated with PCR and PCR-Ddel enzyme digestion technique in 152 cases of DM2.Results:No significant differences in genotype frequencies and C type allele frequency of AT1R gene were found between DM2 -CHD group and DM2 without CHD group(P>0.05). The ACE DD genotype (51.5% vs 32.2%,P<0.05 ) and D typo allele frequencies (68.2% vs 56.3%, P<0.05) were prevalent in DM2-CHD group. Logistic regression analysis showed that ACE DD genotype, BMI and serum total cholesterol level were risk factors of DM2 complicated with CHD. Conclusion:The AT1R gene Al166C polymorphism is not associated with CHD in DM2 patients. ACE gene I/D polymorphism plays a role in the pathogenesis of DM2 complicated with CHD in China. It is suggested that ACE genotypes determining is helpful to screen out susceptible patients.
