^(99m)Tc-HL91显像在肺癌诊断中的应用 被引量:6

The Clinical Value of ^(99m)Tc-HL91 Imaging in the Diagnosis of Pulmonary Cancer

摘要 目的:探讨99mTc-HL91显像对肺癌诊断的临床价值。方法:对37例疑为肺癌的肺部病变患者进行胸部99mTc-HL91显像。静脉注射99mTc-HL91925MBq后1h、4h行平面显像,4h加做断层显像。利用感兴趣区(ROI)技术,计算靶/非靶比值(T/N),并对良恶性病变的T/N值进行比较。结果:临床病理证实肺癌26例,肺部良性病变11例,在1h及4h平面、4h断层图像的T/N值分别为1.26±0.16、1.42±0.26、2.05±0.77及1.07±0.31、1.14±0.20、1.44±0.42。两组间在相应各时间点的T/N值均有显著差异(P<0.02)。1h、4h平面、4h断层显像诊断肺癌灵敏度、特异性、准确性分别为69.2%、72.7%、70.3%;88.5%、72.7%、83.8%;92.3%、72.7%、86.5%。结论:99mTc-HL91具有明显亲肺恶性肿瘤的特性,其显像对肺癌的诊断具有一定的临床价值,显像时间为注射显像剂后4h为佳,且断层显像优于平面显像。 Objective:To evaluate the clinical value of 99mTc-HL91 scintig raph y in the diagnosis of pulmonary cancer. Methods: Thirty seven patients with susp ected pulmonary cancer underwent thoracic 99mTc-HL91 planar imaging at 1h and 4 h and all patients were also performed SPECT at 4h after the intravenous inject ion of 925 MBq 99mTc-HL91.T(the tumor)/N(contralateral normal lung tissue)value was calculated and T/N value in malignant foci was compared with benign lesion. Results: Twenty six patients with pulmonary cancer and 11 cases with benign lesi ons were enrolled in this study. The T/N values of benign and malignant lesions at 1 h and 4 h planar images and 4h SPECT slices were 1.07±0.31,1.14±0.20,1. 44±0.42 and 1.26±0.16,1.42±0.26,2.05±0.77,respectively, there were signif icant difference between both lesions in time of each point(P< 0.02). The sensit ivity, specificity and accuracy in differentiating malignant and benign lesions with 1 h and 4 h planar images and 4 h SPECT slices were 69.2%,72.7%,70.3% ;88.5%,72.7%,83.8%;92.3%,72.7%,86.5%,respectively. Conclusion: 99mT c-HL91 scintigraphy is an excellent modality in the diagnosis of pulmonary canc er. 99mTc-HL91 exhibits a good imaging characteristics,especially at 4 h, it is further enhanced by SPECT.
