无锡市监管场所羁押人员HIV感染状况与危险因素分析

Analysis on Prevalence and Risk Factors of HIV Infection Among Detainees in Detention Places in Wuxi City

目的分析无锡市监管场所羁押人员人类免疫缺陷病毒(HIV)感染状况及危险因素,为制定防治策略提供依据。方法对辖区11家监管场所2011-2015年羁押人员进行HIV相关行为学调查并开展HIV抗体筛查,筛查阳性者送无锡市艾滋病确认实验室确认;运用SPSS18.0对数据进行统计分析。结果无锡市监管场所羁押人员HIV感染率为0.19%;不同户籍羁押人员HIV感染率差异有统计学意义(P=0.00);有危险行为羁押人员HIV感染率(1.52%)高于无危险行为人员(0.33%)(χ~2=911.29,P=0.00);其中有性乱史的羁押人员HIV感染率最高(2.21%);羁押人员中新发现的HIV/AIDS迟发现比例(7.63%),低于2015年无锡市全人群新发现HIV/AIDS迟发现比例(31.82%)。结论无锡市监管场所羁押人员中存在一定的HIV感染流行,性乱与注射吸毒仍是主要的传播途径,应重点加强针对来自艾滋病高疫情省份并且有高危行为的男性青壮年人群开展有效的行为干预,提高HIV抗体检测率。

Objective To analyze the prevalence and risk factors of human immunodeficiency virus(HIV) infection among detainees in detention places in Wuxi,and to provide basis for the prevention and control of HIV/AIDS. Methods The detainees from 2011 to 2015 in 11 custody places of Wuxi were investigated on their HIV-related behaviors. The serum samples were collected for HIV serological test and the positive samples were confirmed by HIV antibody confirmation in Wuxi AIDS confirmation laboratory. The data were processed with SPSS18. 0. Results Among the detainees in Wuxi,HIV-positive rate was 0. 19%. Significant difference of HIV-positive rate was found in detainees with different household registers(P = 0. 00). The HIV-positive rate in the high risk group was higher(1. 52%) than that in non-high risk group(0. 33%)(χ~2= 911. 29,P = 0. 00). The HIV-positive rate in the detainees with promiscuity history was highest(2. 21%). The late discovery ratio of newly-found HIV/AIDS infections(7. 63%) was lower than that of newly-found HIV/AIDS infections in whole Wuxi in 2015(31. 82%). Conclusion There is HIV prevalence among detainees in detention places in Wuxi promiscuity and injecting drug use are still the main route of transmission. It should be focused on strengthening effective behavior intervention among male young adults from HIV/AIDS epidemic provinces with high-risk behaviors,and increasing HIV antibody detection rate.
