血浆同型半胱氨酸水平与扩张型心肌病患者慢性心力衰竭的相关性 被引量:8

摘要 目的探讨血浆同型半胱氨酸(Hcy)水平与扩张型心肌病(扩心病)患者慢性心力衰竭的关系。方法连续性纳入2015年1月至2017年12月住院治疗的扩心病致慢性心力衰竭患者87例进行回顾性分析。按纽约心功能分级分为3组,按血浆Hcy水平分为高Hcy组和正常Hcy组。检测血浆Hcy、脑钠尿肽、多普勒超声心动图测量的左心室舒张末期内径(LVEDD)、左心房前后径(LAD)、左心室射血分数(LVEF)和左心室舒张早期最大血流速度/二尖瓣环平均舒张早期运动速度(E/e’),观察扩心病心力衰竭患者血浆Hcy水平与纽约心功能分级、脑钠尿肽、LVEDD、LAD、LVEF、E/e’的相关性。结果随着纽约心功能分级增加,Hcy水平逐渐增高(P<0.05)。高Hcy组的LVEF低于正常Hcy组[(32.6±8.1)%比(41.4±6.5)%,P<0.05]。Hcy与脑钠尿肽(r=0.273,P<0.05)、LVEDD(r=0.559,P<0.05)呈正相关,与LVEF(r=-0.482,P<0.05)呈负相关。多因素线性回归分析显示,校正年龄、性别等影响因素后,Hcy是LVEDD与LVEF的影响因素(分别B=0.390和-0.489,均P<0.01)。结论扩心病心力衰竭患者高Hcy血症发生率较高,其Hcy水平与纽约心功能等级、LVEF、脑钠尿肽、LVEDD相关。
