超声造影在结肠癌术前分期的临床研究 被引量:4

摘要 目的探讨超声造影用于结肠癌术前分期的临床价值。方法对于诊断结肠癌的32例患者采用回声造影剂充盈结肠后经腹进行超声扫查,与术后肿瘤组织病理分期进行比较,判断进行结肠癌术前分期的可靠性。结果术前超声造影与术后病理诊断相比较,肿瘤T1、T2、T3、T4期符合率分别为100.0%、90.9%、100.0%、92.3%,总符合率为93.8%。其中,T1、T3、T4期诊断符合率较高,T2期较低。对有无淋巴结转移的判断N0期符合率80.3%,N1期符合率100.0%,总符合率为93.8%。结论超声造影可作为结肠癌术前检查及临床分期的重要手段,为治疗方法、手术方式的选择提供有力证据。 Objective To investigate the potential role of contrast-enhanced ultrasonography(CEUS) in the preoperative staging of colonic cancer.Methods Thirty-two patients with known colonic cancer were referred for abdominal ultrasonography after sonographic contrast enema.Results Staging of colonic cancer according to the diagnostic criteria by ultrasonography:uT1 means the tumor confined to submucosa(1 patient);uT2 means tumor invasion of muscularis propria(11 patients);uT3 means tumor invasion through bowel wall(7 patients);uT4 means tumor involving adjacent structures(13 patients).Compared with the pathologic results,accuracy for tumor depth of sonographic contrast enema-enhanced abdominal ultrasonography was 87.50%.Contrast-enhanced ultrasonography had a high degree of accuracy for T1,T3 and T4 tumors,and a low degree of accuracy for T2 tumors.The accuracy in assessment of lymph node metastasis for N0 and N1 were 80.3% and 100%,respectively.Conclusion Contrast-enhanced ultrasonography is one of the important tools for preoperative staging of colonic cancer,providing strong evidence for the surgical planning.
