TightRope装置治疗Rockwood Ⅲ型及以上急性肩锁关节脱位患者的效果分析

Effect Analysis of TightRope Device in Treatment of Patients with Rockwood Type Ⅲ and above Acute Acromioclavicular Joint Dislocation

目的:探析TightRope装置治疗Rockwood Ⅲ型及以上急性肩锁关节脱位患者的治疗效果。方法:选取2016年1月-2018年3月于本院骨外科就诊的60例确诊急性肩锁关节脱位患者。按照随机数字表法将其分为试验组和对照组,各30例。对照组给予关节镜下复位肩锁钩钢板内固定,试验组给予TightRope装置治疗。对比两组患者肩关节复位疗效、手术和术后各临床指标及术后并发症的发生情况。结果:试验组肩关节功能优良率高于对照组(P<0.05);试验组平均手术时间、住院时间与肩关节功能恢复时间均短于对照组,术后引流量少于对照组,比较差异均有统计学意义(P<0.05);试验组术后并发症发生率低于对照组(P<0.05)。结论:TightRope装置治疗Rockwood Ⅲ型及以上急性肩锁关节脱位的效果较好,能够尽早地恢复肩关节功能,改善肩关节的活动度,明显降低术后并发症的发生,且患者容易接受,值得临床推广应用。

Objective:To explore the therapeutic effect of TightRope device in treatment of patients with Rockwood type Ⅲ and above of acute acromioclavicular joint dislocation.Method:A total of 60 patients with acute acromioclavicular joint dislocation diagnosed in our hospital from January 2016 to March 2018 were selected.According to the random number table method,they were divided into experimental group and control group,30 cases in each group.The control group was treated with arthroscopic reduction and internal fixation with acromioclavicular hook plate,and experimental group was treated with TightRope device.The curative effect of shoulder joint reduction,clinical indexes and complications after operation between two groups were compared.Result:The total excellent rate of shoulder joint function in experimental group was significantly higher than that of control group(P<0.05).The average operation time,hospitalization time and recovery time of shoulder joint function in experimental group were significantly less than those of control group,the postoperative drainage volume was significantly less than that of control group,the differences were statistically significant(P<0.05).The incidence of complications in experimental group was significantly less than that of control group(P<0.05).Conclusion:TightRope device has a good effect in treatment of acute acromioclavicular joint dislocation of Rockwood type Ⅲ and above.It can restore shoulder function as early as possible,improve shoulder joint mobility,significantly reduce the incidence of postoperative complications,and is easy for patients to accept.It is worthy of clinical application.
