宁神温胆汤治疗以阴性症状为主的精神分裂症疗效及对血浆中GABA和Glu含量的影响

Therapeutic Effect of Ningshen Wendan Decoction on Schizophrenia Patients with Predominantly Negative symptoms and Its Influence on GABA and Glu Contents in Plasma

目的:探讨宁神温胆汤治疗以阴性症状为主的精神分裂症患者的疗效及对患者血浆中γ-氨基丁酸(GABA)和谷氨酸(Glu)的影响。方法:146例以阴性症状为主的精神分裂症患者分为观察组和对照组,每组73例。观察组给予利培酮联合宁神温胆汤治疗,对照组采用利培酮单药治疗。观察治疗8周后两组的疗效、阴性与阴性症状量表(PANSS)评分、治疗期间的不良反应、血浆中GABA和Glu的含量。结果:观察组有效率为91.78%,明显高于对照组的83.56%(P<0.05)。治疗8周后,观察组PANSS阳性因子分为(11.56±2.67)分、一般精神病理分为(18.65±2.64);对照组为(11.02±3.34)分和(18.34±3.90)分,组间比较无明显差异(P>0.05)。观察组PANSS阴性因子为(26.24±3.21)分、总分为(56.21±10.04)分,对照组分别为(32.26±2.97)分和(62.35±8.34),组间比较有明显差异(P<0.05)。治疗期间,观察组头晕和代谢综合征的发生率明显低于对照组(P<0.05)。观察组血浆中GABA浓度和Glu浓度分别为(0.52±0.10)nmol/L和(76.25±6.81)nmol/L,对照组分别为(0.82±0.14)nmol/L和(66.32±4.27)nmol/L,组间比较明显差异(P<0.05)。结论:宁神温胆汤能够增强利培酮改善精神分裂症阴性症状的效果,并且能降低头晕和代谢综合征的发生。宁神温胆汤治疗精神分裂症的疗效与血浆中GABA浓度的降低和Glu浓度的升高有关。

Objective:To investigate the effect of Ningshen Wendan Decoction on schizophrenia patients with predominantly negative symptoms and its influence on GABA and Glu contents in plasma.Methods:A total of 146 schizophrenic patients with predominantly negative symptoms were divided into observation group and control group,73 cases in each group.The patients in observation group were treated with risperidone combined with Ningshen Wendan Decoction,while the control group was treated with risperidone.The curative effect,negative and negative symptom scale(PANSS)score,adverse reactions during treatment,and the levels of GABA and Glu in plasma of the two groups were observed after 8 weeks treatment.Results:The effective rate of the observation group was 91.78%,which was significantly higher than 83.56%of the control group(P<0.05).After 8 weeks treatment,the scores of PANSS positive factors in the observation group were(11.56±2.67)points,and the general psychopathological scores were(18.65±2.64)pointsand(11.02±3.34)points and(18.34±3.90)points respectively in the control group.There was no significant difference between the two groups(P>0.05).The scores of PANSS negative factor in the observation group was(26.24±3.21)points and the total score was(56.21±10.04)points,and the control group’s socres were(32.26±2.97)points and(62.35±8.34)points,respectively.There was significant difference between the two groups(P<0.05).During the treatment period,the incidence of dizziness and metabolic syndrome in the observation group was significantly lower than that in the control group(P<0.05).The concentration of GABA and Glu in the plasma of the observation group were(0.52±0.10)nmol/L and(76.25±6.81)nmol/L,respectively,while those of the control group were(0.82±0.14)nmol/L and(66.32±4.27)nmol/L,respectively.There were significant differences between the two groups(P<0.05).Conclusion:Ningshen Wendan Decoction can enhance the effect of risperidone on improving negative symptoms of schizophrenia,and reduce the occurrence of dizziness and metabolic syndrome.The therapeutic effect of Ningshen Wendan Decoction on schizophrenia is related to the decrease of GABA concentration in plasma and the increase of Glu concentration.
