尿液甘氨酰脯氨酸二肽氨基肽酶测定在高血压患者肾脏早期损害的诊断价值

摘要 目的 探讨测定尿液甘氨酰脯氨酸二肽氨基肽酶 (glycyl- proline dipeptidyl am inopeptidase,GPDA)在高血压患者肾脏早期损害的诊断价值。方法 采用连续监测法测定尿液 GPDA,速率散射比浊法检测尿液微量白蛋白 (m AL B) ,全定量酶免疫法测定尿液视黄醇结合蛋白 (RBP) ,Jaffe速率法测定尿液肌酐 (Cr)。结果 高血压病患者尿液 GPDA、m AL B、RBP水平均较对照组增高 ;GPDA的阳性率 (6 4 % )高于 m AL B(5 9% ) /RBP(49% )的阳性率 ,但差异无显著性 ;GPDA和 /或 m AL B 71%、GPDA和 /或 RBP 6 4 %和 m AL B和 /或 RBP6 6 % ,三项阳性率相比差异无显著性 (P>0 .0 5 ) ;GPDA的阳性率与 m AL B+RBP/ GPDA+m AL B/ GPDA+RBP阳性率相比差异无显著性。结论 尿液 GPDA是诊断高血压早期肾脏损害的敏感指标 ,特别是肾小管早期损伤 ,对于高血压肾病的早期诊断、肾功能损害程度判断具有一定的价值。 Objective To probe into the diagnostic value of urine level of glycyl proline dipeptidyl aminopeptidase (GPDA) to early renal injury in hypertension cases.Methods Using continuous monitoring to check the urine GPDA,rate scattering turbidimetry to check urine microalbumin (mALB),total quantitative enzyme immynoassay to check urine retinol binding protein (RBP),and Jaffe′s rate method to check urine creatinine (Cr).Results Hypertension patients all got a higher level of urine GPDA,mALB,and RBP than the control group;the positive rate of GPDA was 64%,which was higher than that of mALB (59%) or that of RBP (49%),but no statistical differences among them;GPDA and/or mALB 71%,GPDA and/or RBP 64%,and mALB and/or RBP 66%,all the above three items showed no significant difference (P>0.05);compared with the positive rates of mALB+RBP/GPDA+mALB/GPDA+RBP,GPDA showed no significant difference in its positive rate.Conclusion Determination of urine GPDA gives a sensitive index to the diagnosis of early renal injury in hypertension cases,especially for diagnosing the early injury of renal tubules;this method is valuable for early diagnosing hypertensive renal disease,and for judging the impairment degree of renal function.
