血液中NSE、D-二聚体变化在新生儿缺氧缺血性脑病中的临床价值 被引量:6

摘要 目的 动态监测新生儿缺氧缺血性脑病时血液中NSE、D 二聚体含量的变化 ,并探讨二者含量与HIE所致脑损伤程度的关系。方法 利用ELISA酶联免疫法和比浊法检测HIE患儿治疗前、后血液中NSE、D 二聚体含量。结果 各观察组在HIE发生第 1天和治疗后第 5天 ,血中NSE和D 二聚体浓度均显著高于对照组 (F =3 .0 2 ,P <0 0 1) ;在治疗 10d后 ,只有重度HIE患儿组血中NSE和D -二聚体浓度仍明显高于对照组 ,差异显著 (F =2 .77,P <0 0 5 ) ,而轻度和中度HIE患儿组血中NSE和D -二聚体浓度已恢复到正常水平。结论 通过检测血液中NSE、D 二聚体浓度有助于早期准确判断HIE患儿脑损伤程度 ,并能帮助预测HIE患儿的预后 。 Objective To detect the variation of neuron specific enolase (NSE) and D dimer in blood from neonates with hypoxic ischemic encephalopathy (HIE), and to explore the correlation of NSE and D dimer with brain damage. Methods NSE and D dimer levels in blood from neonates with HIE were measured by ELISA and turbidimetry before and after therapy. Results Blood NSE and D dimer levels in all objective cases were significantly higher than those in the control cases at 1 d after HIE and 5 d after therapy ( t =2.18-15.17, P <0.01-0.05). Blood NSE and D dimer levels in severity degree cases were significantly higher than those in the control cases at 10 d after therapy ( t =2.07-2.72, P <0.05), but the NSE and D dimer levels recovered to normal in slight and moderate degree cases. Conclusion The determination of blood NSE and D dimer levels can be used to estimate the severity of HIE and to evaluate the prognoses.
