基于“治热以温”辨治热性皮肤病

Treatment of Thermal Skin Diseases Based on “Treating Fever with Warm Medicine”

《素问·至真要大论》中记载:“治热以寒”、“热者寒之”,此皆为清法的实践依据之一。皮肤病多表现为红、肿、热、痛等炎症性皮损,若清热解毒剂早用、滥用、过量会产生诸多流弊,热性皮肤病的施治绝非“清热解毒”四字可以概括。针对“过用寒凉克伐”现象,提出“治热以温”的思想,具体包括:风热犯表,佐以辛温发散,开郁透表;湿热蕴肤,酌加辛温芳香,除湿化浊;邪热郁里,配以辛温宣透,火郁发之;虚热上炎,兼以辛温补阳,引火归元;热瘀互结,伍以辛温行气,气行血行;久热正损,当以甘温扶正,驱邪外出。

It is recorded in Suwen·Zhi Zhen Yao Da Lun that“Treating Fever with cold medicine”and“Heat desease is treated with cold medicine”,which is one of the practical bases of Heat Clearing Method.Skin diseases are often manifested as inflammatory skin lesions such as redness,swelling,heat and pain.If antidotes for heat are used early,abused and overused,there will be many disadvantages.The treatment of hot skin diseases can not be summarized by“clearing heat and detoxifying”.In view of the phenomenon of“excessive use of cold cutting”,the idea of“heat control and temperature control”is put forward,including:wind and heat attack surface,accompanied by heat diffusion,opening stagnation through the surface.Humidity and heat accumulate skin,adding spice and aroma,dehumidify turbidity.Evil heat accumulating in vesera,treated with pungent-warm for drain,fire stagnation requiring dissipation.Deficiency heat on the inflammation,and with pungent-warm tonifying Yang,leading deficiency-heat to the original.Blood stasis and heat interknot,accompany with pungent-warm for promoting the circulation of Qi,Qi and blood moving.Long-time heat has damaged healthy Qi,helping with sweet and warm,expelling evil out.
