腰部核心肌力训练联合疼痛干预对老年腰椎间盘突出症患者腰椎功能、下肢放射痛的影响

The Effect of Lumbar Core Muscle Strength Training Combined with Pain Intervention on Lumbar Function and Lower Limb Radiating Pain in Elderly Patients with Lumbar Disc Herniation

目的探讨腰部核心肌力训练联合疼痛干预对老年腰椎间盘突出症(LDH)患者腰椎功能、下肢放射痛的影响.方法选取我院2019年5月—2023年2月收治的80例LDH患者为研究对象,按随机数字表法将其分为对照组与观察组,每组40例.对照组采用疼痛干预,观察组在对照组基础上采用腰部核心肌力训练.对比两组患者的腰椎功能、下肢放射痛和腰椎活动度、核心稳定性及生活质量.结果干预后,观察组日本骨科学会评分系统各个维度评分均高于对照组,下肢放射痛及腰椎活动度改善情况均优于对照组,单桥耐力试验时间长于对照组,生活质量综合评定问卷各个维度评分均高于对照组,组间差异有统计学意义(P﹤0.05).结论腰部核心肌力训练联合疼痛干预能够有效缓解老年LDH患者的下肢放射痛,提高腰椎功能,并改善患者腰椎活动度及稳定性,提高生活质量.

Objective To explore the effects of lumbar core muscle strength training combined with pain intervention on lumbar spine function and lower limb radiating pain in elderly patients with lumbar disc herniation(LDH).Methods 80 patients with LDH admitted to our hospital from May 2019 to February 2023 were selected as the study objects,and were divided into a control group and an observation group according to random number table method,with 40 cases in each group.The control group received pain intervention,and the observation group received waist core muscle strength training on the basis of the control group.Lumbar function,lower extremity radiative pain and lumbar motion,core stability and quality of life were compared between the two groups.Results After intervention,the scores of the Japanese Orthopaedic Society scoring system in the observation group were higher than those in the control group in all dimensions,the improvement of lower extremity radiative pain and lumbar motion were better than those in the control group,the duration of the single bridge endurance test was longer than that in the control group,and the scores of the comprehensive assessment questionnaire of quality of life in the observation group were higher in all dimensions than that in the control group,with statistical significance(P﹤0.05).Conclusion The combination of lumbar core muscle strength training and pain intervention can effectively alleviate lower limb radiative pain in elderly LDH patients,improve lumbar spine function,improve lumbar range of motion and stability,and enhance quality of life.
