胫后动脉内踝上皮支皮瓣的彩色多普勒超声显像

Color Doppler Flow Imaging in Flaps Based Medial Supramalleolar Branches of the Posterior Tibial Artery

目的探讨彩色多普勒超声(CDFI)显像技术在设计胫后动脉内踝上皮支皮瓣中的应用价值。方法本组18例拟行胫后动脉内踝上皮支皮瓣手术病人,术前均采CDFI探测胫后动脉内踝上皮支于主干发出的部位、动静脉内径、穿皮瓣位置、数量,并予体表标记和记录,与手术对照。结果共探及49条胫后动脉内踝上皮支血管,皮支动脉内径0.5～1.47mm,皮支静脉内径0.6～1.8mm,手术证实CDFI定位准确,18例皮瓣全部成活。结论CDFI显像客观,无创使用,能够准确测定胫后动脉内踝上皮支血管穿出部位和血管内径,对临床皮瓣设计具有重要参考价值。

Objective To explore the value of the color doppler flow （CDFI）imaging in designing flaps based medial supramalleolar branches of the posterior tibial artery. Methods From January 2003 to february 2005, eighteen patients were included in this study. Before operation, CDFI technique was used in every patient for examining the points of the medial supramalleolar branches getting out from the posterior tibial artery and getting in the flap , and their radiuses and numbers were marked and recorded in order to afford reference for designing flaps. Results Forty nine medial supramalleolar branches were examined by CDFI successfully and all the results were confirmed by operation andall flaps survived. Conclusions CDFI is an objective, accurate, non invasive and useful method in designing flaps based medial supramalleolar branches of the posterior tibial artery.
