摘要
目的:在动物多器官衰竭(MOF)模型上动态观察肿瘤坏死因子(TNF)变化,探讨TNF与MOF的内在联系。方法:40只家兔分为3组:A组20只,B、C组各10只。A组用失血性休克、内毒素复合因素复制出MOF模型;B组仅给内毒素;C组仅施失血性休克。衰竭器官≥2个者诊断为MOF。TNF测定用生物法。结果:A组MOF发生率为75%;休克及注射内毒素后TNF水平明显升高,注射内毒素后升高更明显;MOF动物TNF水平明显高于非MOF动物;衰竭器官≥3个的动物明显高于2个器官衰竭的动物;36h以内死亡的动物明显高于36h以后死亡的动物;TNF水平与各主要器官功能变化、MOF发生率、动物存活时间及存活率等均存在显著相关性。结论:TNF在MOF的发生过程中可能起重要作用。
Objective: To observe the dynamic changes of tumor necrosis factor(TNF) in the animal MOF model and probe into the inner link of TNF and multi-organ failure(MOF). Methods: 40 rabbits were divided into 3 groups. As MOF models, 20 animals in group A were given the complex factors of hemorrhagic shock and endotoxin, 10 in group B only given endotoxin and 10 in group C only hemorrhagic shock. MOF was diagnosed if the numbers of the failure organs were 2 or more. TNF was tested by the use of the biomethod. Results : The morbidity of MOF in group A was 75%. The levels of TNF were up after hemorrhagic shock and endotoxin injection ,especially after endotoxin,in the group A; those in the MOF animals were higher than those in the nonMOF; those in the animals with 3 or more failure organs were much higher than those with 2; those in the animals that died within 36 h were distinctly higher than ones that died after 36 h. There was significant interrelationship between the levels of TNF and the functions of main organs, the morbidity of MOF, the number of failure organs, the living time and rate of animals, and so on. Conclusion: TNF may play an important role in the development of MOF.
