全身化疗联合局部热疗治疗晚期胃癌的疗效观察

Efficacy of chemotherapy combined with hyperthermia in the treatment of advanced gastric cancer

目的:研究FOLFOX4方案联合射频热疗治疗晚期胃癌的疗效和不良反应。方法:33例患者接受FOLFOX4方案化疗的同时给以热疗。L-OHP(85-100)mg/m2,静脉滴入2h,d1;CF200mg/m2静脉滴入2h,d1、d2;氟尿嘧啶(5-FU)400mg/m2,静脉推注d1、d2,600mg/m2持续静滴22h,d1、d2;每2周重复,28天为1周期。热疗每周2次,每次60分钟。3个周期化疗后评价疗效。结果:33例患者CR4例(12.1%)、PR13例(39.4%),有效率(CR+PR)51.5%。不良反应主要为胃肠反应、骨髓抑制和感觉神经毒性。结论:FOLFOX4方案化疗联合热疗应用于晚期胃癌疗效肯定,不良反应能耐受。

Objective：To study the response and toxicity of the FOLFOX4 regimen combined with hyperthermia for patients with advanced gastric cancer.Methods： Total of 33 patients with advanced gastric cancer were treated with the FOLFOX4 regimen combined with hyperthermia,oxaliplatin 85-100mg/m^2,2 hours iv drop on d1,calcium foliate 200mg/m^2 iv drop 2 hours on d1,d2,followed by fluorouracil 400mg/m^2 iv on d1,d2 and fluorouracil 600 mg/m^2 iv drop d1,d2,and repeated every 2 weeks,4 weeks was used as 1 cycle.Radio-frequency hyperthermia was used for 60 minutes,2 times every week.Efficacy was evaluated after 3 cycles.Results： Complete responses（CR） were observed in 4 cases（12.1 %）,partial responses in 13 cases（39.4 %）.The total response rate was 51.5%.The main toxicity was nausea /vomiting,bone marrow suppression and neuro-seuxory toxicity.Conclusion： The regimen of FOLFOX4 combined with hyperthermia is effective and tolerable in advanced gastric cancer therapy.
