经尿道电灼电切联合丝裂霉素治疗腺性膀胱炎75例

Transurethral electroresection combined with intravesical instillation of mitomycin in treatment of glandular cystitis： A report of 75 cases

目的：探讨经尿道电灼电切联合丝裂霉素治疗腺性膀胱炎临床疗效。方法：近年来人住我院的并经膀胱镜检查及病理学确诊的腺性膀胱炎患者125例，随机分为两组，对照组50例行经尿道电灼电切术及常规治疗，实验组75例在对照组治疗的基础上术后联合丝裂霉素灌注治疗，随访临床疗效及术后不良反应。结果：本组患者的平均随访时间为10～36（22．74-8．4）个月，实验组治愈64例，好转13例，总有效率85.3％；对照组治愈23例，好转13例，总有效率为72．0％。不良反应中，除低烧发生率外，实验组术后肉眼血尿、排尿困难、尿路刺激症、食欲减退等不良反应发生率均低于对照组，差异具有显著性（P〈0．05）。结论：与单独行经尿道电灼电切手术相比，联合丝裂霉素灌注辅助治疗可有效提高治愈率并降低不良反应发生率。

Objective： To study the clinical curative effect of electroresection transurethral cut combined with intravesical instilla- tion of mitomycin in treatment of gland sex cystitis. Methods： Collected patients with glandular cystitis 125 cases confirmed by cystoscopy inspection and pathological diagnosisin in our hospital in recent years,an divided into two groups randomly. Routine conventional treatment group of 50 cases were treated with transurethral electroresection method and test group of 75 cases were treated combining with intravesical instillation of mitomycin based on the routine treatments following-up clinical efficacy and postoperative adverse reactions. Results： During a mean following-up of 10 - 36 （22.7 ± 8.4） months, 64 cases were healed, 13 cases got improved, and the total effective rate 85.3% in test group; In control gruop 23 cases were healed, 13 cased got improved, the total effective cure rate was 72.0%. Except for the incidence of postoperative fever, the incidences of aderse reactions such as urinary irritationgross and hematuria voiding difficulties are lower than those of the control group, with a significant difference （P 〈 0.05）. Conclusion： Compared with the transurethral electroresection alone in treatment of glandular cystitis, joint mitomycin perfusion treatment can effectively increase the cure rate and reduce the incidence of aderse reactions.
