摘要
目的探讨血常规标本不同保存温度和保存时间对Sysmex XE-2100测定血细胞各参数的影响。方法观察40例K2-EDTA抗凝血标本全血细胞计数(CBC)、白细胞分类计数(Diff)、网织红细胞计数(Retic)各参数在室温和4-6℃保存下不同时段的变化情况,同时结合血涂片、散点图、报警信号等对检测结果进行分析,建立合适的血常规标本保存方法。结果室温条件保存下RBC、HGB、MCH、PLT及RET#在72h内无显著变化,而WBC在72h时显著降低;HCT、MCV、NEUT#随时间呈现较明显的上升趋势;MCHC、LYMPH#、MONO#和EO#呈下降趋势;BASO#由于细胞数量少而差异较大;72h内IRF轻度上升而LFR轻度下降,但均无显著性改变。而其余血细胞参数,虽然4-6℃条件能延缓血细胞形态发生变化的趋势,但对于保存超过24h的标本,其检测结果仅供临床参考。结论血常规标本4-6℃保存对参数影响小于室温保存;对于保存时间在24h到72h的标本,WBC、RBC、HGB、MCH、PLT及RET#的检测结果可靠;由于血细胞形态随着保存时间的延长而发生肿胀变性,其余参数检测结果仅供临床参考。
Objective To study the effects of storage of blood in different temperature and different time on hematologic parameters measured by Sysmex XE-2100.Methods The CBC,Diff and Retic of 40 K2-EDTA anticoagulated blood specimens were detected at 0h,4h,8h,24h,48h and 72h with room temperature and 4~6℃ storage.In order to establishment the suitable storage methods of blood specimens,the changes of hematologic parameters combined with blood smear,scatter plot and flag message were analyzed.Results There were no significant different about RBC,HGB,MCH,PLT and RET# after storage in room temperature with 72 hour,while WBC remained stable in 48 and decreased significantly with 72h.HCT,MCV and NEUT# progressive increased over time,while MCHC,LYMPH#,MONO#,and EO# progressive decreased over time.Basophils,representing the smallest fraction of all the white blood cells,revealed significant variety over time.IRF increased and LFR decreased slightly over time.There were no significant different about WBC,RBC,HGB,MCH,PLT and RET# after storage in 4-6℃ with 72 h.The results of the other hematologic parameters over 24 hours,since blood samples storage in 4~6℃ could retard the blood cells swelling,could be consulted only by clinic.Conclusions The effects of blood storage in 4-6℃ is less than storage in room temperature.clinically reliable results may be obtained for WBC,RBC,HGB,MCH,PLT and RET# from specimens that are up to 24 or 72 hours.
