摘要
目的 :解决纤维胆镜取石治疗胆管、胆囊结石常遇到的困难 ,探讨高能震波对体内结石爆破的安全性和实用性。方法 :在犬胆囊和小肠内放入固醇石、混合石和色素石 ,选择不同的冲击功率放电碎石。结果 :研究了人体体内 3种结石在一定能量下的冲击次数和能量的关系 ,确定不损伤胆囊和小肠内膜的能量安全阈值。在同样的冲击能量下 ,破碎固醇石、混合石和色素石所需的次数不同 ;固醇石所需的次数最少 ,混合石较多 ,色素石最多。结论 :病理检查表明 ,在冲击能量 <4.0J时 ,胆囊和小肠切片粘膜处 ,细胞膜轻度充血 ,出现小出血点。当冲击能量 >4.0J时 ,粘膜有脱落 ,膜下组织水肿。
Objective: In order to solve the difficulties of calculus surgery in application of biliary tract fiberscope. Methods: Put the stones in the gallbladder and small intestine and use different energy to shock them. Results: We studied the effects of high energy shock wave on stone, combination calculus and pigment stone and determined the threshold which can wake them fragmentation but no damage to tissue. There are different effects of highly energy shock wave on the three kinds of stones. With the minimal strike times needed to crush the three kinds of stones were not same when the stones were stricken with same energy. Conclusions: Pathological study shown that the 4.0J was the minimum strength of energy with which respectively the small intestine and gallbladder.
