糖尿病足的综合治疗临床疗效观察

Clinical Effects of Combined Treatment of Diabetic Foot

目的:探讨分析综合治疗方案对糖尿病足(diabetic foot,DF)的临床疗效。方法:选择2009年7月至2012年7月期间在我院我科就诊的糖尿病足病的患者120例,随机分为观察组和对照组两组,对照组采用常规治疗方案,包括控制血糖、抗感染、改善微循环、营养神经,并积极治疗其他并发症,加强对症支持疗法。观察组采用除常规治疗方案,尚联合中药口服及中药外治。通过两组治疗前后所有患者的创面愈合情况、症状评分及踝肱血压指数(ABI)来评估分析两种治疗方案的疗效。结果:观察组的完全愈合率和好转率明显高于对照组,经统计学分析其差异有统计学意义,P<0.05;观察组和对照组在治疗后患者症状评分分级均有好转,观察组好转更明显,其0级占58.33%,1级占33.33%,比较对照组治疗情况,其差异有统计学意义,P<0.05;观察组与对照组ABI值之间的差异无统计学意义,P>0.05,具有可比性;治疗后,其间ABI值之间的差异有统计学意义,P<0.05。结论:综合治疗方案能良好改善患者糖尿病足的血液循环,促进创面愈合,减轻患者足病疼痛、发凉和麻木等症状,改善了ABI值,从而促进了糖尿病足的治疗、改善了其预后。

Objective：To investigate the comprehensive analysis of treatment of diabetic foot （diabetic foot, DF） the clinical curative effect. Methods：During 2009 July to 2012 July selected 120 patients in our hospital were my treatment of diabetic foot disease, were randomly divided into observation group and control group two groups, the control group with conventional treatment, including the control of blood glucose, anti-infection, improving microcirculation, nutrition nerve, and actively treating other complications, symptomatic support therapy. The observation group with conventional treatment, is combined with traditional Chinese medicine for oral and external treatment of traditional Chinese medicine. Through the two groups before and after treatment in al patients with wound healing, symptom score and ankle brachial index （ABI） to evaluate the therapeutic effect of two kinds of treatment. Results：The group of complete healing rate and improvement rate was significantly higher than that of the control group, statistical y significant, the differences of P〈0.05;the observation group and the control group improved symptoms score grading were before and after the treatment, the observation group improved more obviously, the 0 accounted for 58.33%, 1 accounted for 33.33%, compared to the control treatment, with statistical significance, its differences from P〈0.05;the observation group and the control group differences between ABI value was not statistical y significant, P〉0.05, comparable;after treatment, there was a statistical y significant difference between the values of ABI, P〈0.05. Conclusion：Comprehensive treatment can improve the blood circulation of patients with diabetic foot, promote wound healing, reduce the symptoms of patients with foot pain, cold and numb, improve the ABI value, thereby promoting treatment, improve the prognosis of diabetic foot.
