系统应用唑来膦酸对骨质疏松时自体髂骨移植内种植体骨结合影响的影像学及生物力学研究 被引量:2

摘要 目的观察系统应用唑来膦酸对骨质疏松时自体髂骨移植内种植体骨结合的影响。方法 20只家兔分成对照组(A组)、去势组(B组)、系统应用唑来膦酸组(C组),B、C组行去势手术,A组行假手术。术后12周,B、C组家兔行左侧胫骨近中干骺端制作骨缺损,同期行自体髂骨移植术+种植体植入术。自体髂骨移植及种植体植入术后3d,C组家兔开始给予唑来膦酸0.1mg/kg皮下注射,每3周1次。种植体植入术后第12周处死动物,切取标本行X线片检查、显微CT检查及生物力学测试。结果 X线片显示所有标本均正常愈合;显微CT检查及定量数据可见A组植骨块内骨小梁粗大、密集、连续,骨皮质较厚,植骨块和原有骨质之间结合紧密,种植体周围骨组织丰满;B组植骨块及皮质骨较薄,内层骨皮质改建接近消失,骨小梁较稀疏,种植体周围骨组织较少;C组植骨块也较厚,骨小梁密集,骨小梁直径较A组细,种植体周围有较多新生骨小梁,但比A组骨量少。骨组织体积比、骨结合率在A组最高,B组显著低于A组和C组;骨小梁厚度则是A组最高,C组次之,B组最低。A组和C组旋出力矩明显高于B组。结论实验性骨质疏松会使移植骨皮质骨变薄,松质骨量减少,从而降低植骨区种植体的骨性支持,减弱骨组织对种植体的支持;通过应用唑来膦酸涂层种植体,可以改善骨质疏松对种植体周围自体骨植骨愈合的不良影响,增加植骨区内的种植体周围骨量,从而增加其稳定性。
