膀胱内翻性乳头状瘤p53和PCNA的表达及意义

Expressions and Significance of p53 and PCNA in Inverted Papilloma of the Urinary Bladder

[目的]观察膀胱内翻性乳头状瘤(IP)中p53和增殖细胞核抗原(PCNA)的表达及其意义。[方法]对19例膀胱IP、10例浅表性膀胱癌(STCC)、8例浸润性膀胱癌(ITCC)p53、PCNA的表达采用免疫组化Envision二步法进行检测。[结果]在IP中p53和PCNA的阳性细胞表达大多分布于上皮巢的基底层细胞 ,而在STCC和ITCC内的阳性细胞表达呈散在或弥漫分布。其强度p53表达在IP和STCC多呈弱阳性 ,而ITCC多为强阳性。IP组的 p53阳性率明显低于ITCC ,差异有显著性(P<0.01) ,但与STCC组比较无显著差异。PCNA表达强度在IP组与STCC和ITCC组比较无显著差异 ,其阳性率则低于ITCC组 ,有显著性差异(P<0.05) ,而与STCC组比较未显示差异。19例IP中有1例p53和PCNA呈高表达,阳性率分别为31.21%及75.37% ,与ITCC类似。[结论]IP是具有较高增殖活性的良性肿瘤 。

To explore the expressions and significance of p53 and PCNA in inverted papilloma(IP)of bladder.We selected 19 cases with IPs,10 cases with superficial transitional cell carcinoma(STCC),8 cases with invasive transitional cell carcinoma(ITCC)of bladder and evaluated the expression of p53,PCNA by immunohistochemical technique.Positive stainings for p53 and PCNA were observed predominantly in the basal layer epithelial nest in IPs,but scattered or diffuse distribution in STCC and ITCC.The expressions of p53 were low in IP and STCC,and high in ITCC.Positive rate of p53 in IP was significantly lower than that in ITCC(P<0.01).There was no significant difference between IP and STCC.Compared with STCC and ITCC,there were no significant difference in the expression intensity of PCNA in IPs.The positive rate of PCNA in IP was significantly different with that in ITCC,but had no difference with that in STCC.High expressions of p53(31.21%) and PCNA(75.37%)were observed in one of 19 cases with IP.The positive rates were similar to ITCC.[Conclusion]IP is a high proliferative benign tumor.High expressions of p53 and PCNA indicate its susceptibility to malignant transformation or co-existence with carcinoma.
