摘要
目的实验性研究壮通饮(ZTY)对DN大鼠肾小球病理改变的影响。方法采用单肾切除合并腹腔空腹注射链脲佐菌素(STZ),建立DN大鼠模型,并随机分为空白对照组、假手术组、模型组、西药组和壮通饮低剂量组、中剂量组、高剂量组7个组。治疗组每日灌胃,壮药组用壮通饮(低剂量组6.8 g/kg,中剂量组13.6 g/kg,高剂量组27.2 g/kg),西药组用卡托普利1 g/kg。于实验给药治疗6周后,检测各组大鼠血糖、体重、右肾指数,且通过光镜、透射电镜观察各组大鼠肾小球的病理改变。结果与空白对照组比较,假手术组各项指标检测均无明显差异(P>0.05);模型组及各治疗组24 h尿蛋白量、血糖、右肾指数及体重均降低,差异有统计学意义(P<0.05);HE、PAS染色及电镜下,模型组与各治疗组均出现不同程度的病理改变,其中模型组病理改变最明显,肾小球明显肥大、基底膜增宽、系膜区明显增生,毛细血管腔明显变窄,阳性物质明显增多,系膜基质明显增多,肾小球内毛细血管呈节段性硬化,毛细血管管腔狭窄,足细胞的足突融合、消失。与模型组比较,各治疗组24 h尿蛋白量、血糖、右肾指数均降低,差异有统计学意义(P<0.05),而体重虽有所改善,但差异无统计学意义(P>0.05);HE、PAS染色及电镜下,各治疗组病理改变均有不同程度的减轻,其中壮通饮中剂量组减轻最为明显。而各治疗组之间,虽然壮通饮中剂量组各项检测指标及病理改变的减轻、改善程度最为明显,但与其他治疗组相比差异无统计学意义(P>0.05)。结论壮通饮可以改善DN大鼠的一般情况,减轻肾小球病理学改变,对DN大鼠的肾脏具有一定的保护作用。
Objective To experimentally study effect of Zhuang Tong Yin(ZTY)on the pathological changes in glomerulus of diabetic nephropathy (DN) rats. Methods Single kidney nephrectomy combined with injection of streptozotocin (STZ) in fasting abdominal cavity was performed in this study,and model of DN rats was built.All of the DN rats were randomly divided into the following 7 groups : blank control group, sham operation group, model group, western medicine group, ZTY low dose group, ZTY medium dose group and ZTY high dose group.The treatment groups were given intragastric administration every day.The ZTY groups were treated with ZTY (with 6.8 g/kg for the ZTY low dose group, 13.6 g/kg for the ZTY medium dose group,and 27.2 g/kg for the ZTY high dose group). And the western medicine group were given 1 g/kg of captopril. After 6 weeks of experimental treatment, blood glucose, body mass and right kidney index of all the groups were tested.In addition, pathological changes of glomerulus between groups were observed through optical microscopy and transmission e- lectron microscopy.Results Compared with the blank control group, all indicators of the sham group had no statistically sig- nificant differences( P 〉0. 05); the 24 h urine protein,blood glucose,right kidney indexes and body weight of the model group and each treatment group all decreased,and the differences were statistically significant( P 〈0.05) ;HE,PAS staining and transmission electron microscopy showed that: pathological changes in different degrees were found in the model group and each treatment group,in which the pathological changes in the model group were the most obvious-glomerulus enlarged obviously, basement membrane widened significantly, obvious hyperplasia was found in mesangial area, capillary lumen significantly narrowed down,positive substances and mesangial matrix increased significantly, segmental sclerosis was found in glomerular capillary of glomerulus, capillary lumen stenosis was found, and foot processes of podocyte fused and even disappeared.Compared with the model group, the 24 h urine protein,blood glucose and right kidney indexes of each treatment group decreased, and the difference was statistically significant( P 〈0.05), although body mass improved, no statistically sig- nificant difference was found ( P 〉 0.05) ; HE, PAS staining and transmission electron microscopy showed that pathological changes in each treatment group alleviated in varying degrees, among which the ZTY medium dose group alleviated most obviously. Although indicators and pathological changes of the ZTY medium dose group alleviated and improved most obviously, no statistically significant differences were found when comparing with other treatment groups ( P 〉0. 05). Conclusion ZTY can improve general situation of DN rats, reduce pathological changes of glomerulus, and provide certain protection to kidneys of DN rats.
