摘要
目的探讨动态双C疗法在垂体生长激素细胞腺瘤合并血糖异常患者围术期的临床应用效果。方法选择武威市凉州医院2008年3月—2013年3月收治的垂体生长激素细胞腺瘤合并血糖异常患者40例,随机分为治疗组和对照组,每组20例。对照组患者给予指尖血糖测量联合皮下注射胰岛素的常规疗法,治疗组患者给予持续动态血糖监测系统(CGMS)联合动态胰岛素持续注射系统(CSII)的动态双C疗法。比较围术期两组患者空腹血糖(FBG)、餐后2 h血糖(2 h BG)、血糖波动幅度、血糖达标时间、低血糖发生率及胰岛素用量。结果围术期治疗组患者FBG、2 h BG、血糖波动幅度及低血糖发生率低于对照组,血糖达标时间短于对照组,胰岛素用量少于对照组(P<0.05)。结论动态双C疗法在垂体生长激素细胞腺瘤合并血糖异常患者围术期的临床应用效果较好,能有效控制患者围术期血糖、减少胰岛素用量、降低低血糖发生率。
Objective To explore the application effect of continuous dynamic glucose monitoring system( CGMS)combined with dynamic continuous insulin injection system( CSII) on perioperative patients with pituitary growth hormone cell adenoma( PGHCA) complicated with pathoglycemia. Methods From March 2008 to March 2013,a total of 40 perioperative patients with PGHCA complicated with pathoglycemia were selected in the Liangzhou Hospital of Wuwei,and they were randomly divided into treatment group and control group,each of 20 cases. Control group was given finger tip blood glucose measurement combined with subcutaneous insulin infusion,while treatment group was given CGMS combined with CSII. Fasting blood glucose( FBG),2-hour blood glucose( 2 h BG),blood glucose fluctuation,blood glucose control time,incidence of hypoglycemia and insulin dosage were compared between the two groups. Results FBG,2 h BG,blood glucose fluctuation and incidence of hypoglycemia of treatment group was statistically significantly lower than that of control group,respectively,while blood glucose control time was shorter,insulin dosage was less( P〈0. 05). Conclusion CGMS combined with CSII has good application effect on perioperative patients with PGHCA complicated with pathoglycemia,can effective control the blood glucose,reduce the insulin dosage and incidence of hypoglycemia.
