慢性乙型肝炎患者血脂相关指标检测与临床意义

Detection of Serum Lipids of Patients with Chronic Hepatitis B and Its Clinical Significance

目的通过对慢性乙型肝炎(CHB)患者血脂相关指标水平的检测来研究其对肝脏损伤程度的判断。方法随机选取单纯CHB患者40例,乙型肝炎后肝硬化患者30例,40例健康对照者(HC)作为研究对象,检测入选研究对象血脂相关指标:甘油三酯(TG)、总胆固醇(CHO)、高密度脂蛋白胆固醇(HDL-C)、载脂蛋白A_1(ApoA_1)、载脂蛋白B_(100)(ApoB_(100)),肝功相关指标:谷丙转氨酶(ALT)、总胆红素(TBiL)。根据检测结果,回顾性分析血脂各项指标与谷丙转氨酶、总胆红素之间的相关性,探讨血脂指标反映肝脏损害程度。结果单纯慢乙肝、乙型肝炎肝硬化患者的血脂相关指标水平较健康对照者明显降低(P均<0.05),分析患病研究对象血清CHO,TG,HDL-C,ApoA_1,ApoB_(100)与血清ALT,TBiL,发现血脂指标与肝功指标有显著的负相关性(P<0.05)。结论慢乙肝患者血脂相关指标水平明显低于健康人群并能反映肝细胞损害程度,加强慢乙肝患者血清TG,CHO等血脂相关指标的随访检测是有必要的。

Objective To study the judgment of the degree of liver injury through detecting the blood lipid related indexes of chronic hepatitis B （CHB） pations.Methods Randomly selecting 40 patients with pure CHB,30 pa-tients with liver cirrhosis after hepatitis B ,and 40 cases of healthy controls （ HC） as the object of study .Then detecting blood lipid related indexes：triglyceride（TG）,total cholesterol（CHO）,high density lipoprotein（HDL-C）,apolipoprotein A1（ApoA1）,apolipoprotein B100（ApoB100） and relevant indicators of liver function：alanine aminotransferase（ALT）,to-tal bilirubin（TBiL）.According to the detecting results,a retrospective analysis of the correlation between blood lipid in-dexes,glutamic pyruvic transaminase and total bilirubin were made and the relationship between blood lipid index and the degree of liver injury were explored .Results The blood lipid levels of both pure chronic hepatitis B and cirrhosis of hepatitis B patients were significantly different , they were all higher than the blood lipid levels of the healthy control group（P〈0.05）.Then the analyzation of the relevance between serum CHO ,TG,HDL-C,ApoA1,ApoB100 and serum ALT,TBiL of pations infected HBV showed a significant negative correlation between them （P〈0.05）.Conclusion Blood lipid related indexes of chronic hepatitis B patients are significantly lower than that in healthy people and can re -flect the damage degree of liver .It＆39;s necessary to detect and follow-up the blood lipid related indexes including TG ,CHO of CHB patients .
