摘要
目的:评价高频电刀结合排龈线修复老年人龈下楔状缺损的临床效果。方法:选择龈下楔状缺损的患者50例,共211颗患牙,随机分为实验组和对照组,其中110颗纳入实验组,101颗纳入对照组,对照组应用3M Filtek Z250复合树脂常规填充,实验组在高频电刀切龈后,应用排龈线进行排龈,再利用3M Filtek Z250复合树脂进行充填治疗。随访2年,用改良的美国公共健康部制定评定系统(USPHS)临床直接评定标准评价,观察实验组和对照组充填物修复体表面情况、边缘密合性、边缘着色、色泽协调性、继发龋、牙龈炎症和牙髓反应情况。结果:实验组的总疗效(90.3%vs.75.3%)、固位效果(92.2%vs.82.8%)、边缘密合性(82.5%vs.69.9%)、牙髓反应(5.8%vs.18.3%)和继发龋(1.0%vs.4.5%)均明显优于对照组,两组比较,各项差异均有显著性(P<0.05)。结论:高频电刀结合排龈线修复老年人龈下楔状缺损可以明显提高修复体的临床效果。
Objective: To evaluate clinical effects of high frequency electric knife combined with retraction agents on subgingival wedge-shaped defects. Methods: 50 patients with subgingival wedge-shaped defects were selected and were randomly divided two groups. There were 211 teeth, 110 teeth in the experimental group and 101 teeth in the control group,respectively. In the control group the wedge-shaped defects were filled with 3M Filtek Z250 resin. For the experimental group, the gingiva was trimmed using the combination of the high frequency electric knife and the gingival retraction agents, and then 3M Filtek Z250 resin was used to fill the wedge-shaped defects. After 2 years, modified United States Public Health Service(USPHS) guidelines were used to evaluate the restoration surface, marginal sealing, marginal coloration, color coordination, secondary caries occurrence, gingival inflammation and pulp reaction in both control and experimental groups. Results: The general curative effect(90.3% vs. 75.3%), retention rate(92.2% vs. 82.8%), marginal sealing(82.5% vs. 69.9%), pulp reaction(5.8% vs. 18.3%) and secondary caries occurrence(1.0% vs. 4.5%) in the experimental group were significantly better than those in the control group(P〈0.05). Conclusions: Utilization of high frequency electric knife combined with retraction agents in the restoration of subgingival wedge-shaped defects can efficiently improve the overall quality of the restoration.
