48例鼻缺损修复

Forty-eight cases of nasal defect repair

目的总结各种鼻缺损修复的临床效果,提供临床应用经验。方法对自2008至2014年就诊于中国医科大学附属口腔医院的48例鼻部缺损的患者,分别采用全厚皮片、耳郭复合组织瓣、眉间旋转皮瓣、鼻唇沟瓣、额部皮瓣、前臂挠侧游离组织皮瓣修复,并分析其缺损原因、缺损部位、缺损大小、修复方式及愈后情况。结果随访6-24个月,其中5例皮瓣臃肿,2例术后瘢痕明显,1例感染,3例部分皮瓣坏死,2例外形不对称。其余患者术后愈合情况较好,对美观及功能恢复满意。结论对于不同部位、不同大小的缺损应采取不同的修复方式,根据患者的缺损情况制定相对恰当的治疗方案,能较好恢复患者鼻部的功能及美观。

Objective To summarize the clinical effects of various types of nasal defect repair,providing experience for clinical application.Methods From 2008 to 2014,48 patients with nasal defects were collected from the Hospital of Stomatology,China Medical University.The reason for the defect,the defect site,the defect size and repair methods were analyzed.The repair methods included：full thickness skin graft,auricle compound tissue flap,glabellar flap,nasolabial flap,forehead flap and radial forearm free flap.Results All patients were followed up for 6 to 24 months.All were satisfied with the aesthetic contour and functional recovery except for 5 patients with a bloated flap,2 cases with postoperative scar,1 case with infection,3 cases with partial flap necrosis and 2 cases with asymmetry.Conclusion The defects in different sites and with different sizes should be repaired in different ways.Appropriate treatments were selected according to the various defects in order to achieve good function and aesthetic effect.
