心理支持护理对肺癌术后化疗患者自我感受负担和生活质量的影响

Effect of psychological support on self-perception of burden and quality of life in patients undergoing postoperative chemotherapy for lung cancer

目的探讨心理支持护理对肺癌患者自我感受负担和生活质量的影响。方法选取2014年1月至2015年10月间陕西省宝鸡市中心医院收治的100例肺癌术后化疗患者,采用随机数字表法分为观察组与对照组,每组50例。观察组患者在对照组基础上采用心理支持护理干预,对照组患者采用常规护理方法,12周后,对比两组患者心理状态、生活质量和自我感受负担。结果干预后,观察组患者的焦虑自评量表(SAS)和抑郁自评量表(SDS)评分明显低于对照组,两组比较,差异均有统计学意义(均P<0.05)。观察组患者的自我感受负担(SPB)总分及各项目情感性负担、身体性负担、社交性负担、发展受限负担和时间依赖负担得分均明显低于对照组,两组比较,差异均有统计学意义(均P<0.05)。观察组患者和对照组患者生活质量(KPS)评分各项目均高于对照组,两组比较,差异有统计学意义(P<0.05)。结论心理支持护理使肺癌患者减少了焦虑和抑郁情绪的产生,改善了患者自我感受负担,提高了生活质量。

Objective To study the effect of psychological support on self-perception of burden and quality of life in patients undergoing postoperative chemotherapy for lung cancer.Methods From January 2014 to October 2015,100 patients with lung cancer undergoing postoperative chemotherapy at Baoji Central Hospital were selected and divided into an observation group and a control group with 50 patients in each group.The observation group was given psychological support intervention based on conventional nursing care and the control group was given conventional nursing care.After 12 weeks,anxiety scale（SAS） and self-rating depression scale（SDS） were used to assess the degree of anxiety and depression in the two groups before and after the intervention.Using Karnofsky performance status（KPS） score,quality of life was measured.Self-perception of burden was measured using care giver burden inventory（CBI） scale.Results After the intervention,SDS and SAS scores were significantly lower in the observation group than in the control group（P〈0.05）.Overall CBI,emotional burden,physical burden,sociability burden,burden of development restriction and time-dependent burden scores were significantly lower in the observation group than in the control group were in the control group（all P〈0.05）.The score of all the items for KPS scale was higher in the observation group than in the control group（P〈0.05）.Conclusion The psychological supportive care can reduce anxiety and depression,improve self-perception of burden,and accordingly improve quality of life in patients with lung cancer.
