摘要
目的:观察人迎穴改良针刺法对椎动脉型颈椎病(CSA)患者颈部血流速度的影响及临床疗效。方法:59例CSA患者随机分为常规组30例、改良组29例。常规组采用人迎穴常规针刺法,改良组采用人迎穴改良针刺法,均每日治疗1次,6d为1个疗程,治疗2个疗程。治疗前后分别测定经颅多普勒中左椎动脉(LVA)、右椎动脉(RVA)、基底动脉(BA)的收缩峰血流速度(Vs)及颈性眩晕症状与功能评估量表评分,并观察临床疗效。结果:两组患者治疗后颈性眩晕症状与功能评估量表评分,LVA、RVA、BA的Vs与治疗前比较,均明显改善(P<0.01,P<0.05),且改良组优于常规组(P<0.05)。改良组总有效率为93.1%(27/29),常规组为70.0%(21/30),改良组疗效优于常规组(P<0.05)。结论:人迎穴改良针刺法用于治疗CSA能有效增快椎-基底动脉的血流速度,改善局部血液循环,缓解患者病痛。
Objective To observe the clinical efficacy of modified acupuncture at Renying point (ST 9) for patients with cervical spondylosis of vertebral artery type and its influence on velocity of cervical blood flow. Methods Fifty-nine cases of vertebral artery type cervical spondylosis were randomly divided into control group (n = 30) and treatment group (n = 29). Both groups were acupunctured at ST 9, with routine acupuncture technique used in the control group and modified technique in the treatment group, respectively. All cases received two courses of treatment, each course covered consecutive 6 once-per- day treatments. Before and after treatment, transcranial Doppler (TOD) was used to measure the systolic peak blood flow veloci- ty (Vs) of left vertebral artery (LVA), right vertebral artery (RVA) and basilar artery (BA), and the scores of "cervical vertigo symptoms and functional assessment scale" (OVSFA8) were also assessed, separately. Results CVSFAS scoring, Vs of LVA, RVA and BA after treatment showed significant improvement compared with those before treatment (P〈0.01, P〈0.05). The efficacy of the treatment group in the above mentioned indexes was superior to that of the control group (P〈0.05). The total effective rate of the treatment group was 93. 1% (27/29), superior to 70. 0% (21/30) of the control group (P〈0. 05). Conclusion The modified acupuncture method at ST 9 is clinically effective in the treatment of cervical spondylosis of vertebral artery type via increasing the Vs of vertebral-basilar artery, improving the local blood circulation and relieving pain.
