摘要
为研究湖北省黄石地区女性人乳头瘤病毒(HPV)感染状况及其亚型分布,回顾性分析应用导流杂交基因芯片技术检测从2016年9月至2017年7月2 648例女性宫颈拭子HPV基因分型的结果,比较不同基因型的流行病学特点,并采用SPSS 17.0软件对数据进行统计分析。结果为:在2 648例检测标本中,检测出HPV阳性感染率为43.92%(1 163/2 648)。感染年龄分布以>60岁年龄段感染率最高,为63.46%;其次是≤20岁年龄段(47.56%);而后是21~30岁年龄段(47.39%),51~60岁年龄段(44.69%),41~50岁年龄段(42.87%),31~40岁年龄段(40.76%),各年龄组感染率之间无统计学差异(P>0.05)。20种HPV基因亚型中,居前5位的分别是HPV52(20.64%)、HPV16(14.70%)、HPV58(10.83%)、HPV6(6.45%)、HPV33(5.85%)。在不同宫颈病变的HPV感染状况中:宫颈炎组(24.38%)、CIN Ⅰ组(48.70%)、CIN Ⅱ组(77.89%)、CIN Ⅲ组(89.64%)、宫颈癌组(93.23%),两两比较有统计学意义(P<0.01)。单一型别占总感染例数的75.49%(878/1 163);多重感染率为24.51%(285/1 163),其中双重感染率为17.45%(203/1 163),三重以上混合感染率为7.05%(82/1 163)。湖北省黄石地区女性HPV感染率较高,应加强对HPV的筛检力度,为HPV相关疾病的防治提供重要参考。
Objective : To investigate infection status and distribution of cervical human papillomavirus (HPV) in huangshi,hubei province. Methods:All 2,648 cases were studied. Cervical cell specimens and 20 hpv geno-types were analyzed by PCR and reverse dot blot gene chip technology. Results : Total of 1 163 specimens were found with positive HPV among 2 648 cases,the positive rate was 43. 92% (1 163/2 648 ) . The infec-tion rate in 〉60 years old was the highest(63.46% ) ,next was2 0 years old(47.56% ) ,21 ~30 years old (47. 39% ) ,51 -60 years old(44. 69% ) ,41-50 years old(42. 87% ) ,31 ~40 years old(40. 76% ) . How-ever, there was no satistical difference among age groups (P 〉0.05) . 20 kinds of HPV gene subtype, the top five was respectively HPY52(20.64% ) ,HPY16 (14.70% ) ,HPY58( 10. 83% ) ,HPY6 (6.45% ) ,HPY33 (5. 85%). Different cervical lesions of HPY infection status were cervicitis group (24. 38%) , CIN I (48.70% ),CIN II group(77. 89% ),CIN III group(89. 64% ) ’cervical cancer(93. 23% ),multiple com- parison among these was statistically significant( P 〈0.01) . The infection rate of single subybe was 75.49% (878/1 163) , Multiple infection rate was 24. 51% (285/1 163). With dual infection in 17. 45% (203 / 1 163) ,more than triple mixed infection rate was 7. 05% (82/1 163 ) . Conclusion; HPY infection rate is higher in Huangshi city,the HPY screening should be done and strengthened,which can provide a valuable reference for the prevention and treatment of HPY related disease.
