口腔扁平苔藓组织中CD3^+、CD8^+T细胞浸润与不典型增生的关系 被引量:6

摘要 目的 :探讨口腔扁平苔藓(OLP)中CD3^+T细胞和CD8^+T细胞浸润与OLP伴上皮不典型增生的相关性,为OLP的临床诊断和免疫治疗提供新的方向。方法:采用免疫组化半定量法检测CD3^+、CD8^+T细胞在79例OLP患者组织中的表达,取10例口腔正常黏膜组织作为对照,分析口腔扁平苔藓组织中CD3^+、CD8^+T细胞浸润与不典型增生的关系。结果 :CD3^+、CD8^+T细胞在正常口腔黏膜组织中呈阴性表达,在79例OLP中均呈阳性表达。CD3^+、CD8^+T细胞浸润与患者性别、年龄、发生部位无明显统计学意义,但是与OLP伴上皮不典型增生密切相关,具有统计学意义(P<0.001,P=0.002),CD3^+T细胞浸润和CD8^+T细胞浸润之间具有显著相关性(r=0.498,P<0.001)。结论:口腔扁平苔藓患者组织中CD3^+、CD8^+T细胞浸润与上皮不典型增生密切相关,影响OLP的发生发展。 Objective:To investigate the relationship between CD3~+T cells and CD8~+T cells infiltration and atypical hyperplasia in oral lichen planus(OLP),which will provide a new direction for the clinical diagnosis and immunotherapy of OLP.Methods:Immunohistochemical semi-quantitative method was used to detect the CD3~+T cells and CD8~+T cells infiltration in 79 cases of OLP patients,meanwhile 10 cases of normal oral mucosa as a control.The relationship between these T cell infiltration and atypical hyperplasia in OLP was analyzed by statistical methods.Results:CD3~+T cells and CD8~+T cells were negatively expressed in normal oral mucosa,but were positively expressed in 79 cases of OLP.The infiltration of CD3~+T cells and CD8~+T cells had no relationship with gender,age or the location of OLP,but they were closely correlated with the atypical hyperplasia of OLP(P 0.001 and P=0.002,respectively),the infiltration of CD3~+T cells were positively associated with the infiltration of CD8~+T cells(r=0.498,P 0.001).Conclusions:The infiltration of CD3~+T cells and CD8~+T cells is closely related to epithelial dysplasia,which affects the occurrence and development of OLP.
