摘要
目的探讨苯扎氯铵(benzalkonium,BZK)对血管吻合手术模型大耳白兔动脉血液内皮素(endothelin,ET)、一氧化氮(nitric oxide,NO)浓度的影响及意义,阐明不同浓度BZK溶液与血管壁刺激之间的关系。方法选取48只新西兰大耳白兔,随机分为对照组(生理盐水组)和BZK溶液0.01%、0.05%、0.1%组。制作血管吻合手术实验动物模型。检测分析各组实验动物术后血液ET、NO浓度的变化。结果手术后各组内不同时点ET水平均呈逐渐降低趋势,但差异无统计学意义(P>0.05);而在同一时点中各组ET水平呈逐渐升高趋势,术后6hC组和D组高于A组和B组,术后12hD组高于A组和B组,术后24hD组高于A、B、C组,差异均有统计学意义(P<0.05)。手术后各组内不同时点NO水平均呈逐渐升高趋势,A组、B组、C组差异有统计学意义(P<0.05),D组差异无统计学意义(P>0.05);而在同一时点中各组NO水平呈逐渐降低趋势,术后6h、12hC组和D组低于A组和B组,术后24hB、C、D组低于A组,差异均有统计学意义(P<0.05)。结论血管吻合术中,在血管断端应用0.01%BZK溶液进行冲洗、消毒,其血管壁刺激性明显小于高浓度BZK溶液,可减轻发生血管痉挛的程度,提高血管吻合口处的血流通畅度,减少血管危象发生,提高血管吻合手术的成功率。
Objective To study the effect and the significance of benzalkonium(BZK)chloride on blood endothelin(ET)and nitric oxide(NO)concentrations of rabbits surgical model of vascular anastomosis of BZK chloride on vascular anastomosis in rabbits model of dynamic blood ET and NO and the significance of the effect of concentration,and to elucidate the relationship between different concentration of BZK chloride solution and the degree of stimulation of vascular wall.Methods The 48 New Zealand white big-ear-rabbits were divided randomly into control group(saline group),BZK chloride solution 0.01%,0.05%,0.1%group.An experimental animal model of vascular anastomosis was established.The changes of blood ET and NO concentration in the experimental animals were detected and analyzed.Results After the operation,the levels of ET in each group were decreased gradually,but the difference was not statistically significant(P〈0.05).At the same time,the level of ET was increased gradually in each group.By 6 hafter operation,it was higher in group C and D than in group A and B.By 12 h after operation,it was higher in group D than in group A and group B.By 24 hafter operation,it was higher in group D than in group A,B,C,the difference was statistically significant(P〈0.05).At different time points after operation,the level of NO was increased gradually.There was a significant difference in group A,group B and group C(P〈0.05),but there was no significant difference between group D(P〉0.05).At the same time point,the NO level of each group was decreased gradually.By 6 hand 12 hafter operation,it was lower in group C and D than group A and B.By 24 hafter operation,it was lower in group B,C and D than group A,and the difference was statistically significant(P〈0.05).Conclusion The consistency of BZK chloride for vascular anastomosis surgery should be 0.01%,which can reduce the stimulation to vascular wall,reduce vascular spasm after the surgery,help the wound end unobstructed,and prevent blood vessel from danger.
