摘要
目的检测原发性干燥综合征(pSS)患者外周血CD19+CD5+B淋巴细胞及淋巴细胞亚群水平,探讨其在pSS疾病发生发展中及病情活动监测中的作用。方法选择73例确认原发性干燥综合征患者作为pSS组,40健康体检患者作为对照组。采用流式细胞方法检测CD19+CD5+B淋巴细胞及淋巴细胞亚群,免疫透射比浊法检测免疫球蛋白G(IgG),全自动血沉分析仪法检测红细胞沉降率(ESR)。由专科医师对pSS患者进行疾病活动指数(ESSDAI)评分。不同组间淋巴细胞亚群检测结果比较采用Mann-Whitney U检验,相关分析采用Spearman相关系数检验进行统计学分析。结果 pSS组IgG、ESR、CD19+细胞、CD19+CD5+B淋巴细胞结果明显高于对照组(P<0.05),CD4+T细胞、CD4+/CD8+比值pSS组低于对照组(P<0.05)。CD19+CD5+细胞与ESR、IgG及ESSDAI评分水平均呈正相关(相关系数分别为0.40、0.48、0.53,且P<0.05),CD4+T细胞与ESR、IgG结果呈负相关(相关系数分别为-0.38、-0.35,且P<0.05)。结论原发性干燥综合征患者血液中存在淋巴细胞亚群数量紊乱,外周血CD19+CD5+B淋巴细胞检测有助于原发性干燥综合征疾病活动期的辨别。
Objective To determine the levels of CD19+CD5+B cell and lymphocyte subsets in patients with primary Sj9 gren′s Syndrome(pSS)and investigate the role of them in the pathogenesis of pSS.Methods 73 primary Sj9 gren′s Syndrome patients were enrolled as pSS groups and 40 healthy volunteers were selected as control groups.CD19+CD5+B cell and lymphocytes subsets was detected by FCM(Flow cytometry),IgG was detected by Immunological transmission turbidimetry.ESR was measured by Automatic ESR Analyzer.Disease Activity Index(ESSDAI)of primary Sj9 gren′s Syndrome patients were evaluated by physicians.MannWhitney U was used to compare the results of CD19+CD5+B cell and lymphocyte subsets between different groups.Correlation Analysis was conducted by Spearman Method between lymphocyte subsets and ESSDAI、IgG、ESR.Results High levels of IgG、ESR、CD19+、CD19+CD5+and low levels of CD4+/CD8+were found in pSS groups comparing with control groups(P0.05).There were positive correlation between CD19+CD5+and IgG、ESR、ESSDAI respectively.Negative correlation was found between CD4+cells and IgG、ESR respectively.The other lymphocyte subsets had no correlation with IgG、ESR、ESSDAI.Conclusion Lymphocytes subsets disorder often occur in patients with pSS.The levels of CD19+CD5+B cell are closely associated with the activity of pSS.
