PHILOS钢板治疗肱骨近端骨折的效果分析

The Effect Analysis of Proximal Humeral Fracture with PHILOS Plate

目的:分析PHILOS钢板治疗肱骨近端骨折的临床效果。方法:随访2013-2016年笔者所在医院65例PHILOS钢板治疗肱骨近端骨折的患者,女30例,男35例,平均年龄(55.6±2.5)岁,NEER分型Ⅱ型9例,Ⅲ型40例,Ⅳ型16例,术后对其肩关节功能constant评分进行分析。结果:所有患者均获得随访,随访时间15~28个月,平均(19.8±3.9)个月,根据末次复查X线结果,骨折均愈合,无螺钉及钢板断裂,1例患者出现肱骨头坏死。按照constant评分,优32例,良20例,中9例,差4例,优良率80%。结论:PHILOS钢板治疗肱骨近端骨折,锁定螺钉分布合理,固定效果可靠,预后优良率较高,临床治疗效果确切,值得推广。

Objective：To explore the effect of the PHILOS plate in the treatment of proximal humeral fracture.Method：Sixty-five patients from the anther＇s hospital,30 females and 35 males,average age（55.6±2.5）years,with proximal humeral fractures treated with PHILOS plate from 2013-2016.There were 9 cases of Type Ⅱ,40 cases of Type Ⅲ,16 cases of Type Ⅳ,according to the NEER classification.The postoperative constant scores of the shoulder joint were observed.Result：All patients were followed up from 15 to 28 months,average（19.8±3.9）months.X-Ray was reexamined after operation and all the patients were bony union,no plate loosening or breakage,but one case with humeral head necrosis.According to constant score,excellent in 32 cases,good in 20 cases,medium in 9 cases,fair in 4 cases,the excellent-good rate was 80%.Conclusion：PHILOS plate is effective in the treatment of proximal humeral fractures,with reasonable distribution of locking screws,reliable fixation,high prognosis and accurate clinical treatment.
