干性型老年性黄斑变性的局部视网膜电图振荡电位

摘要 目的:为了研究黄斑功能和黄斑活动状况的客观测量并对老年性黄斑变性(AMD)的进一步理解。方法:对干性型老年性黄斑变性69例和正常人不同部位的黄斑48例进行检测黄斑区的局部振荡电位(FOPS)和局部视网膜电图(FERG)的a波及b波。刺激光斑分别用3°、5°、10°与15°的蓝光、绿光及白光。结果:老年性黄斑变性患者在5°黄斑区域内FOPs振幅降低及波节减少与视力具有显著相关、FOPs振幅降低与对照组有高度显著差异(P<0.01),a波与b波振幅的降低与对照组亦存在显著差异(P<0.05),10°刺激光斑下,a波、b波与FOPs振幅的降低均具有显著意义(P<0.05);而在15°刺激光斑下,仅FOPs振幅的降低存在显著差异(P<0.05)。一组早期AMD患者表现为OP_3与OP_4子波振幅的明显降低(P<0.01);而另一组的改变则以OP_1与OP_2为主(P<0.05)。另外,16眼AMD患者全部子波均降低。结论:黄斑区OPs的记录可较客观而灵敏地反映AMD的循环状况,并对中心视功能的客观评估具有一定意义。 In order to investigate the macular function and provide an objective measure of macular activities and further understanding in age-related macular degeneration (AMD).Methods: Macular focal oscillatory potentials (FOPs) as well as a-and b-wave of electroretinogram were recorded from 69 patients with the dry type of AMD and 48 normal control subjects. The spot light stimuli were 3°,5°, 10° and 15° macular regions respectively. Results. When compared to controls, the amplitudes of FOPs were reduced significantly obviously (P&lt;0.01), while a-and b- wave were also reduced significant (P&lt;0.05)for.3°.and 5°spot;FOPs amplitudes as well as a- and b-wave were reduced significantly for 10° spot (P&lt;0.05).OPs amplitudes were also reduced selectively (P&lt;0.05)leaving a- and b-wave intact for 15° spot. OP3 and OP4 were selectively reduced in a group of early AMD patients (35 eyes), while OP1 and OP2 reduced in the other group (18 eyes). In addition, all of the wavelets were reduced in 16 eyes. Conclusion. It would be of clinical value to obtain a focal OPs test result that correlates with the AMD patient’s central visual function.
