摘要
目的探讨超敏C反应蛋白(high-sensitivity C reactive protein,hs-CRP)对缺血性心脏病患者冠状动脉微血管功能障碍的预测价值。方法选择本院2017年1月至2018年12月收治的缺血性心脏病患者80例为研究对象,根据是否存在冠状动脉微血管功能障碍将其分为障碍组(48例)和正常组(32例),观察两组患者hs-CRP水平和其他临床特征的差异,同时采用ROC曲线分析hs-CRP对缺血性心脏病患者冠状动脉微血管功能障碍的预测价值。结果障碍组患者hs-CRP水平显著高于正常组(P < 0.001)。多因素Logistic回归分析显示:hs-CRP水平升高、高脂血症、左前降支血流量减少均是缺血性心脏病患者冠状动脉微血管功能障碍的危险因素(均P < 0.05)。hs-CRP诊断缺血性心脏病患者冠状动脉微血管功能障碍的曲线下面积为0.798(95%CI: 0.701 ~ 0.896, P < 0.001)。hs-CRP水平与缺血性心脏病冠状动脉微血管功能障碍患者同型半胱氨酸水平、颈动脉内膜中层厚度均呈正相关(均P < 0.05),与左室射血分数、左前降支血流量均呈负相关(均P < 0.05)。结论 hs-CRP对预测缺血性心脏病患者冠状动脉微血管功能障碍具有较高价值。
Objective To investigate the predictive value of high-sensitivity C reactive protein(hs-CRP) in coronary microvascular dysfunction in patients with ischemic heart disease. Method 80 patients with ischemic heart disease treated in our hospital from January 2017 to December 2018 were selected as subjects, according to the existence of coronary microvascular dysfunction, they were divided into dysfunction group(n = 48) and normal group(n = 32), the differences of hs-CRP level and other clinical features between the two groups were observed, and the predictive value of hs-CRP for coronary microvascular dysfunction in patients with ischemic heart disease was analyzed by ROC curve. Result The level of hs-CRP in normal group was significantly higher than that in normal group(P < 0.001). Multivariate Logistic regression analysis showed that the increase of hs-CRP level, hyperlipemia and reduction of blood flow in left anterior descending branch were the risk factors of coronary microvascular dysfunction in patients with ischemic heart disease(all P < 0.05). The area under the curve of coronary microvascular dysfunction in patients with ischemic heart disease diagnosed by hs-CRP was 0.798(95%CI: 0.701 ~ 0.896, P < 0.001). The level of hs-CRP was positively correlated with the level of homocysteine and carotid intima-media thickness in patients with coronary microvascular dysfunction complicated with ischemic heart disease(all P < 0.05), and it was negatively correlated with left ventricular ejection fraction and left anterior descending branch blood flow(all P < 0.05). Conclusion Hs-CRP has high value in predicting coronary microvascular dysfunction in patients with ischemic heart disease.
