千金苇茎汤加味联合头孢哌酮舒巴坦对老年人肺炎疗效观察及对血白细胞、中性粒细胞、超敏C反应蛋白的影响

Observation on curative effects of modified Qianjin Weijing decoction combined with cefoperazone sulbactam in treatment of elderly patients with pneumonia and its effect on blood leukocyte,neutrophils and high-sensitivity C-reactive protein

目的探讨千金苇茎汤加味联合头孢哌酮舒巴坦对老年人肺炎疗效观察及对血白细胞、中性粒细胞、超敏C反应蛋白的影响。方法选取2016年3月~2018年3月入住我院的老年肺炎患者142例,按治疗方式分为两组,各71例,对照组仅予以头孢哌酮舒巴坦治疗,研究组予以千金苇茎汤加味联合头孢哌酮舒巴坦治疗,比较两组治疗有效率、住院时长、临床症状消失用时、出现不良反应的概率以及WBC(白细胞数)、NEUT%(中性粒细胞比例水平)、hs-CRP(超敏C反应蛋白)水平变化情况。结果对照组有效率为81.7%,低于研究组的97.2%,差异有统计学意义(P<0.05);对照组出现胃肠反应以及肝功能异常、血细胞异常的不良反应概率高于研究组,差异有统计学意义(P<0.05);治疗前两组WBC、NEUT%、hs-CRP水平差异无统计学意义(P>0.05),治疗后对照组WBC、NEUT%、hs-CRP水平均高于研究组,差异有统计学意义(P<0.05);对照组住院时长、发热、咳嗽、啰音、咳痰消失时长均高于研究组,差异有统计学意义(P<0.05)。结论在治疗老年人肺炎的过程中,千金苇茎汤加味联合头孢哌酮舒巴坦可提高治愈率,缩短住院时长,降低不良反应,值得在临床上推广与应用。

Objective To explore curative effects of modified Qianjin Weijing decoction combined with cefoperazone sulbactam in treatment of elderly patients with pneumonia and its effect on blood leukocyte,neutrophils and high-sensitivity C-reactive protein.Methods 142 elderly patients with pneumonia who were admitted to our hospital from March 2016 to March 2018 were selected and they were divided the control group and the study group according to the treatment methods,with in 71 cases each group.The control group was treated with cefoperazone sulbactam while the study group was treated with Qianjin Weijing decoction combined with cefoperazone sulbactam.The effective rate of treatment,length of hospital stay,disappearance time of clinical symptoms,probability of adverse reactions and changes in WBC(white blood cell count),NEUT%(neutrophil ratio)and hs-CRP(high-sensitivity C-reactive protein)levels were compared between the two groups.Results The effective rate of treatment in the control group was 81.7%,lower than that of the study group(97.2%),and the difference was statistically significant(P<0.05).The incidence of adverse reactions such as gastrointestinal reactions,abnormal liver function and abnormal blood cells in the control group was higher than that in the study group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in WBC,NEUT%and hs-CRP levels between the two groups before treatment(P>0.05).After treatment,WBC,NEUT%and hs-CRP levels in the control group were all higher than those in the study group(P<0.05).The length of hospital stay and disappearance time of fever,cough,rales and expectoration of the control group were all higher than those in the study group,and the difference was statistically significant(P<0.05).Conclusion In the treatment of elderly patients with pneumonia,modified Qianjin Weijing decoction combined with cefoperazone sulbactam can improve the cure rate,shorten the length of hospital stay and reduce adverse reactions,which is worthy of clinical promotion and application.
