牙周修复联合治疗对前牙残根残冠修复的美学及生物学指标的影响

Aesthetic and Biological Effects of Periodontal Repair Combined Therapy on Residual Crown Restoration of Anterior Teeth

目的:探究牙周修复联合治疗对前牙残根残冠修复的美学及生物学影响。方法:选取笔者医院2017年3月-2018年6月收治的不良修复体致前牙区残根残冠患者25例。采用桩核冠联合牙冠延长术进行治疗,从修复成功率、牙周指标、美学评价、龈沟液炎症因子水平变化等方面,评价该方案的治疗效果。结果:成功修复25例,修复成功率100.00%。术后患者的牙周指标:牙周探诊深度(PD)、附着水平(AL)、龈沟出血指数(SBI)以及龈沟液炎症因子[肿瘤坏死因子-α(TNF-α)、白细胞介素-1β(IL-1β)、NO、细胞间粘附分子-l (ICM-l)]均呈逐渐下降的趋势,单因素方差分析显示下降程度明显。术后VAS评分、PES评分和WES评分较修复前有明显提高,患者美学修复满意率100.00%。结论:桩核冠联合牙冠延长术用于不良修复体致前牙区残根残冠患者的疗效确切,既保留了患牙牙根,又有效恢复了患者前牙区的形态美观。还能有效改善牙周炎症情况,有利于维护牙周健康。

Objective To explore the aesthetic and biological effects of periodontal repair combined therapy on residual roots and crown restoration of anterior teeth.Methods 25 patients with anterior teeth residual roots and crown by malfunctional prostheses from March 2017 to June 2018 in our hospital.Treated with post-core crown and crown lengthening surgery.The therapeutic effects of the regimen were evaluated in terms of repair success rate,periodontal index,aesthetic evaluation,and changes in inflammatory factors of gingival crevicular fluid.Results Successfully repaired 25 cases,the success rate of repair was 100.00%.Postoperative periodontal parameters:PD,AL,SBI and gingival fluid inflammatory factors TNF-α,IL-1β,NO,ICM-l showed a gradual decline trend,one-way ANOVA showed a significant degree of decline.The postoperative VAS score and PES score and WES score were significantly higher than those before the repair.Patient aesthetic restoration satisfaction rate 100.00%.Conclusion The effect of post-core crown combined with crown lengthening surgery for patients with residual roots and crown by malfunctional prostheses in anterior teeth is good.It not only retains the root of the tooth,but also effectively restores the appearance of the patient's anterior region.It can also effectively improve periodontal inflammation and help maintain periodontal health.
