摘要
目的探讨全瓷冠修复牙体缺损的效果及对患者龈沟液中白细胞介素-8(IL-8)、基质金属蛋白酶8(MMP-8)、肿瘤坏死因子-α(TNF-α)的影响。方法选取我院2017年1月至2017年5月收治的100例上颌前牙牙体缺损患者,采用随机数字表法分为A组和B组各50例,A组患者采用全瓷冠修复治疗、B组采用钴铬合金烤瓷冠修复;对比两组修复前后的牙周探诊深度(PD)、修复效果、牙龈指数(GI)、龈沟液及其中IL-8、MMP-8和TNF-α的水平。结果治疗前,A组和B组的GCF、PD及GI值,差异无统计学意义(P>0.05);治疗后,A组患者的GCF、PD及GI值显著的低于B组患者,差异有统计学意义(P<0.05);治疗前,A组和B组的龈沟液中IL-8、MMP-8和TNF-α的水平,差异无统计学意义(P>0.05);治疗后,A组患者的龈沟液中IL-8、MMP-8和TNF-α的水平显著的低于B组患者,差异有统计学意义(P<0.05);治疗6个月后,A组患者的修复体颜色匹配、边缘适合性均优于B组,差异有统计学意义(P<0.05)。结论全瓷冠修复牙体缺损的效果较好,可降低龈沟液中IL-8、MMP-8和TNF-α的水平,对患者牙周组织的影响更小。
Objective To investigate the effect of all-ceramic crown on repairing tooth defects and its effects on interleukin-8(IL-8),matrix metalloproteinase 8(MMP-8),tumor necrosis factor-α(TNF-α)ingingivalcrevicularfluid.Methods From January 2017 to May 2017,100 patients with maxillary anterior tooth defects treated in our hospital were selected and divided into A group and B group by using random number table method.A group was treated with all-ceramic crowns,while B group was repaired with cobalt-chromium alloy porcelain crown.The periodontal depth(PD),repair effect,gingival index(GI),gingival crevicular fluid and its IL-8,MMP-8 and TNF-alpha level were compared.Results Before treatment,there was no significant difference in GCF,PD and GI values(P>0.05).After treatment,the GCF,PD and GI valuesof A group were significantly lower than those of B group,and the differenceswere statistically significant(P<0.05).Before treatment,the levels of IL-8,MMP-8 and TNF-αin the gingival crevicular fluid were not statistically significant(P>0.05).After treatment,IL-8,MMP-8 and TNF-αin the gingival crevicular fluid of A group were significantly lower than those of B group,and the differences were statistically significant(P<0.05).At 6 months after treatment,in A group,the color matching and edge suitability of the restorationswere better than those of B group,and the differences were statistically significant(P<0.05).Conclusion All-ceramic crowns could have a good effect on repairing tooth defectsby reducing IL-8,MMP-8 and TNF-αin gingival crevicular fluid and have less effect on the periodontal tissue of patients.
