改良局麻注射法减少眼周表皮囊肿皮下瘀血手术1例

A case of improved local anesthesia injection to reduce subcutaneous hemorrhage in periocular epidermal cyst

表皮囊肿(Epidermal Cyst)是由鳞状上皮构成的囊壁和内含层状角化物质构成。因眼周皮肤薄且皮下组织疏松,毛细血管分布丰富,传统局部麻醉注射法易导致眼周淤血、血肿,影响手术视野及切口愈合,且后期造成的色素沉着影响切口美观。现报道我科治疗的1例眼周表皮囊肿的年轻女性患者,术中采用精准的局部麻醉注射法,手术1周后随访切口周围无淤血、水肿、色素沉着,术后2个月切口恢复良好且同时达到美容修复效果。

Epidermal Cyst is composed of squamous epithelium in cyst wall and contains lamellar keratinous substances.Due to the thin skin around eyes,loose subcutaneous tissue and rich capillary distribution,traditional local anesthesia injection can easily lead to subcutaneous hemorrhage and hematomas,which will affect the surgical field and wound healing,and the pigmentation caused in the later stage will affect the incision aesthetics.Herein,a young female patient presented with epidermal cysts around the eye who treated with improved local anesthesia injection was reported as follow,No subcutaneous hemorrhage,edema,and pigmentation were observed around the incision one week after the operation,the incision recovered well and achieved cosmetic repair effect after 2 months later.
