射频电刀联合中药外敷治疗肥大增生型酒渣鼻的临床疗效观察

Clinical Efficacy Observation of Radiofrequency Electrotome Combined with External Application of Traditional Chinese Medicine in the Treatment of Hypertrophic Rosacea

目的观察射频电刀微雕整形修复联合中药治疗肥大增生型酒渣鼻的安全性和有效性。方法将确诊为肥大增生型酒渣鼻的患者38例,随机分为射频刀A组(13例)、射频刀B组(13例)和普通电刀组(12例)。射频刀A组采用射频电刀微雕整形修复+复方紫草油纱,射频刀B组采用射频电刀微雕整形修复+抗生素油纱,普通电刀组采用普通电刀切除增生鼻赘后再用多功能电离子治疗仪塑形+抗生素油纱。对比分析3组手术时间、术中出血量、术后创面愈合率及术后创面愈合时间。结果3组病例全部完成治疗,与普通电刀组比较,射频刀组手术时间明显缩短(23.85±1.21,23.69±1.18比45.92±1.165)min、术中出血量明显减少(10.54±0.88,10.38±0.87比38.58±0.90)mL,射频刀A组在术后第10天创面愈合率(56.44±3.15%)、第15天创面愈合率(97.10±4.05)%及创面愈合时间(14.92±1.11)d,明显优于射频刀B组[(53.93±3.97)%,(89.57±4.11)%,(16.31±0.86)d]及普通电刀组[(52.51±3.67)%,(88.26±3.92)%,(16.67±0.89)d],差异有统计学意义(P<0.05)。3组术后第5天创面愈合率及术后瘢痕发生率比较差异无统计学意义。结论射频电刀微雕整形方法能安全有效治疗肥大增生型酒渣鼻,较好恢复鼻部形态,相对于普通电刀的手术所需时间短,术中出血少,术后联合复方紫草油纱外敷创面能缩短创面愈合时间,提高创面愈合率。

Objective In order to observe the safety and efficacy of radiofrequency electrotome micro plastic surgery combined with traditional Chinese medicine(TCM)in the treatment of hypertrophic rosacea.Methods Thirty-eight patients diagnosed with hypertrophic rosacea were randomly divided into three groups,radiofrequency electrotome group A(n=13),radiofrequency electrotome group B(n=13)and common electrotome group(n=12).Radiofrequency electrotome group A adopted radiofrequency electrotome in combination with compound Zicao oil yarn.Radiofrequency electrotome group B adopted radiofrequency electrotome in combination with antibiotic ointment.In the common electrotome group,after the removal of the hypertrophic nasal tissue by common electrotome,the nose shape was recreated with multifunctional electronic therapeutic apparatus,and antibiotic ointment was applied in the end.Analysis was based on the comparison of the following aspects among the three groups,operation time,intraoperative blood loss,postoperative wound healing rate and postoperative wound healing time.Results All the patients completed the treatment.Compared with common electrotome group,the operation time of the radiofrequency electrotome groups was significantly shortened(23.85±1.21,23.69±1.18 vs.45.92±1.165)min,and the intraoperative blood loss was significantly reduced(10.54±0.88,10.38±0.87 vs.38.58±0.90)mL.The wound healing rate on the 10th and 15th day[(56.44±3.15)%,(97.10±4.05)%]and the wound healing time[(14.92±1.11)d]of the radiofrequency electrotome group A were significantly better than those of the radiofrequency electrotome group B[(53.93±3.97)%,(89.57±4.11)%,(16.31±0.86)d]and the common electrotome group[(52.51±3.67)%,(88.26±3.92)%,(16.67±0.89)d],with statistically significant differences(P<0.05).There were no statistically significant differences in wound healing rate on the 5th day after surgery.Conclusion Radiofrequency electrotome can treat hypertrophic rosacea safely and effectively.It can restore nasal morphology better,shorten operation time,and led to less intraoperative bleeding.Furthermore,the wound healing time can be shortened and the healing rate can be improved when combined with TCM compound Zicao oil yarn after the operation.
