四川省二、三级医院肿瘤日间化疗病房开设需求调查研究

Investigation and research on the demand of setting up tumor daytime chemotherapy wards in secondary and tertiary hospitals in Sichuan province

目的调查四川省肿瘤日间化疗病房开设现状及需求,为其开设提供依据及支持。方法通过自行设计的肿瘤日间化疗病房开设需求调查表,对四川省21个市州64所二、三级医院进行调查。结果四川省仅有15所二、三级医院开设肿瘤日间化疗病房。未开设肿瘤日间化疗病房的49所医院中,26.53%认为开设非常重要,48.98%认为比较重要;16.33%认为非常迫切,36.74%认为比较迫切。开设肿瘤日间化疗病房患者获益:缓解住院难(89.80%),降低医疗费用(87.76%),提高患者满意度(71.43%);医院获益:减少医院住院药占比(81.63%),提高床位使用率(77.55%),优化管理流程(57.14%)。开设制约因素:部分异地医保门诊无法报销(73.47%),医院无整体规划(61.22%),医务人员和患者对日间化疗认知低(46.94%)。结论四川省二、三级医院对肿瘤日间化疗病房开设有较大需求,但目前受到多种因素制约,建议相关部门提供有力支持,促进其开设和发展。

Objective To investigate the status quo and demand of setting up tumor daytime chemotherapy wards in Sichuan province,and to provide basis and support for its setting up.Methods A survey was conducted in 64 secondary and tertiary hospitals in 21 cities and prefectures of Sichuan province through a self-designed questionnaire on the demand of setting up tumor daytime chemotherapy wards.Results Only 15 secondary and tertiary hospitals in Sichuan province have set up tumor daytime chemotherapy wards.Among the 49 hospitals that did not set up the tumor daytime chemotherapy wards,in 26.53%hospitals it was thought very important and in 48.98%hospitals it was thought fairly important,while in 16.33%hospitals it was thought very urgent and in 36.74%hospitals it was thought fairly urgent.The benefits to patients for setting up tumor daytime chemotherapy wards included relieving the difficulty of hospitalization(89.80%),reducing medical expenses(87.76%)and improving patient satisfaction(71.43%).The benefits to hospitals for setting up it included reducing the proportion of inpatient drugs in hospital(81.63%),increasing the utilization rate of beds(77.55%),and optimizing the management process(57.14%).The restrictive factors of setting up tumor daytime chemotherapy wards included part of the medical outpation service can not be reimbursed(73.47%),lack of overall planning in hospitals(61.22%),and insufficient cognition of daytime chemotherapy of medical staff and patients(46.94%).Conclusion There is a great demand for setting up tumor daytime chemotherapy wards in secondary and tertiary hospitals in Sichuan province,but it is restricted by many factors at present.It is suggested that relevant departments provide strong support to promote its setting up and development.
