即刻种植术联合个性化修复基台治疗前牙缺失疗效观察

Observation on the curative effect of immediate implantation combined with personalized repair abutment in the treatment of anterior tooth loss

目的探讨即刻种植术联合个性化修复基台治疗前牙缺失的效果。方法将106例(167颗患牙)前牙缺失患者按治疗方法分为两组,对照组(50例,79颗患牙)行延期种植术联合个性化修复基台治疗,研究组(56例,88颗患牙)拔除前牙行即刻种植术联合个性化修复基台治疗,比较两组种植情况及修复满意度、美学效果(红色美学指数、白色美学指数评分)、口腔健康状况(菌斑指数、牙龈指数及出血指数)及生活质量(口腔健康影响程度量表评分)。结果两组种植成功率及修复满意度比较差异无统计学意义(P>0.05)。修复1个月、3个月、6个月末研究组红色美学指数、白色美学指数评分显著高于对照组(P<0.05或0.01)。修复6个月末研究组菌斑指数、牙龈指数及出血指数显著低于对照组(P<0.05)。修复3个月、6个月末研究组口腔健康影响程度量表评分显著低于对照组(P<0.01)。结论即刻种植术联合个性化修复基台治疗前牙缺失效果确切,能改善口腔健康状况,达到更好的美学效果,提高患者生活质量。

Objective To investigate the effect of immediate implantation combined with personalized repair abutment in the treatment of anterior tooth loss.Methods A total of 106 patients(167 affected teeth)with anterior teeth loss were divided into two groups according to the treatment method.The control group(50 cases,79 affected teeth)received delayed implantation combined with personalized repair abutment,and the study group(56 cases,88 affected teeth)received immediate implantation combined with personalized repair abutment before removing the anterior teeth.The implant situation and repair satisfaction,aesthetic effect[pink esthetic score(PES),white esthetic score(WES)],oral health status[plaque index(PLI),gingival index(GI),bleeding index(BI)]and quality of life[14-item oral health impact profile(OHIP-14)]were compared between the two groups.Results There were no significant differences in the success rate of implantation and repair satisfaction between control group and study group(P>0.05).The PES and WES scores in study group at 1 month,3 months and 6 months after repair were significantly higher than those in control group(P<0.05 or 0.01).The PLI,GI and BI at 6 months after repair in study group were significantly lower than those in control group(P<0.05).The OHIP-14 score of the study group was significantly lower than that of the control group at the end of 3 and 6 months of repair(P<0.01).Conclusions Immediate implantation combined with personalized repair abutment has a definite effect on the treatment of anterior teeth loss,which can improve the oral health status,achieve better aesthetic effect and improve the quality of life of patients.
