自拟通络化痰方联合奥卡西平片治疗缺血性卒中后癫痫(痰瘀互结证)的疗效及对癫痫发作、血清指标的影响

Effect of Using Self-made Tongluo Huatan Decoction Combined with Oxcarbazepine Tablets in the Treatment of Epilepsy after Ischemic Stroke(Phlegm and Blood Stasis Syndrome)and Its Influence on Epileptic Seizures and Serum Indexes

目的:观察给予缺血性卒中后癫痫(痰瘀互结证)自拟通络化痰方联合奥卡西平片治疗对其癫痫发作、血清指标的影响作用。方法:选取2017年1月~2019年1月本院收治的86例缺血性卒中后癫痫患者,采用随机数字法将其分为观察组(n=43)和对照组(n=43),对照组给予奥卡西平片单药治疗,观察组在对照组基础上联合自拟通络化痰方治疗,比较两组临床疗效、血清神经元特异性烯醇化酶(NSE)、肿瘤坏死因子-α(TNF-α)、白细胞介素-1β水平、美国国立卫生院神经功能缺损(NIHSS)评分、简易智力状态检查量表(MMSE)评分、日常生活能力(ADL)评分以及1年内癫痫发作情况。结果:观察组总有效率、MMSE评分、ADL评分均高于对照组(P<0.05);观察组NSE、TNF-α、IL-1β水平和NIHSS评分均低于对照组(P<0.05);观察组1年内癫痫发作次数少于对照组(P<0.05)。结论:给予缺血性卒中后癫痫(痰瘀互结证)自拟通络化痰方联合奥卡西平片治疗临床效果好,可明显改善患者血清NSE、TNF-α、IL-1β水平和神经功能,提升日常生活能力、减少癫痫发作次数,有利于其恢复。

Objective:To investigate the effect of using Self-made Tongluo Huatan Decoction combined with Oxcarbazepine Tablets in the treatment of epilepsy after ischemic stroke(phlegm and blood stasis syndrome)and the influence on epileptic seizures and serum indexes.Methods:A total of 86 patients with epilepsy after ischemic stroke admitted to the hospital from January 2017 to January 2019 were selected and divided into an observation group(n=43)and a controlled group(n=43)by the random number method.The controlled group were treated with Oxcarbazepine Tablets,while the observation group were treated with Self-made Tongluo Huatan Decoction on the basis of the controlled group.The clinical effects,serum neuron-specific enolase(NSE),tumor necrosis factor-α(TNF-α)and interleukin-1β(IL-1β)levels,the National Institutes of Health Stroke Scale(NIHSS)scores,Mini Mental State Examination Scale(MMSE)scores,Activity of Daily Living Scale(ADL)scores,and epileptic seizureswithin 1 year were compared between the two groups.Results:The total effective rate,MMSE score,and ADL score of the observation group were higher than those of the controlled group(P<0.05),the levels of NSE,TNF-αand IL-1β,and NIHSS scorewere lower than those of the controlled group(P<0.05),and the number of epileptic seizures within 1 year was smaller than that in the controlled group(P<0.05).Conclusion:Self-made Tongluo Huatan Decoction combined with Oxcarbazepine Tablets is effective in the treatment of epilepsy after ischemic stroke(phlegm and blood stasis syndrome),which can significantly improve the patients’serum NSE,TNF-αand IL-1βlevels,neurological function and activities of daily living,and reduce epileptic seizures,thereby promoting recovery.
