本文总结了曹志群教授治疗难治性胃食管反流病的临证经验,主要从病因病机及具体治法两个方面进行阐述。曹志群教授在长期的临证中总结出难治性胃食管反流病的发生与郁火密切相关。气机郁滞是难治性胃食管反流病的发病基础,因郁成火,火热炎上,胃失和降而为病。治疗以火郁发之为原则,针对郁火之因,需先开通郁闭;针对郁热之果,在辛散透达的基础上加以清热,同时注重滋养阴液;针对郁滞状态,注重调理脾胃气机。临证具体从以下三个方面论治:开郁闭,通结滞;清郁热,养阴液;调脾胃,降逆气。

This paper summarizes the clinical experience of Professor Cao Zhiqun in the treatment of refractory gastroesophageal reflux disease,and elaborates on two aspects of etiology and pathogenesis,and specific treatment experience.Professor Cao Zhiqun has concluded that the occurrence of refractory gastroesophageal reflux disease is closely related to stagnation fire in his long-term clinical practice.The basis of refractory gastroesophageal reflux disease is qi depression which causes the stomach to lose its balance.It is considered that the treatment should follow the principle of release of fire stagnation,to deal with the cause of stagnation fire,opening the stagnant blockage is the first step.In response to the results of the heat of stagnation,the method of clearing heat is added to the method of dispersing and clearing heat,while taking care of nourishing yin fluid.For the stasis,he focuses on regulating spleen and stomach qi.Clinical treatment is specifically based on three aspects,opening and clearing up stagnation,clearing the heat of stagnation,and nourishing yin fluid,making spleen and stomach get the balance.
