不同OLGIM分期胃炎患者PGⅠ、PGR水平变化及HP感染状况分析 被引量:5

Changes of pepsinogen Ⅰ levels, pepsinogen ratio, and HP infection in gastritis patients with different OLGIM stages

摘要 目的 研究不同可操作的与胃癌风险联系的肠化生评估(OLGIM)分期胃炎患者胃蛋白酶原Ⅰ(PGⅠ)、胃蛋白酶原比值(PGR)水平及幽门螺杆菌(HP)感染状况。方法 回顾性选取2019年1月至2021年1月西安高新医院消化内科收治的120例胃炎患者纳入观察组,同期选择在我院体检的110例健康人群作为对照组。比较两组受检者的血清PGI水平、PGR及HP感染状况,并比较不同OLGIM分期胃炎患者的血清PGⅠ水平、PGR及HP感染状况,采用Pearson相关性分析法分析血清PGI水平、PGR及HP感染阳性率与OLGIM分期的相关性。结果 观察组患者的血清PGⅠ水平、PGR分别为(74.89±3.71) ng/mL、5.16±1.01,明显低于对照组的(95.40±5.85) ng/m L、7.43±1.60,Hp感染阳性率为66.67%,明显高于对照组的13.64%,差异均有统计学意义(P<0.05);四组OLGIM分期患者的血清PGⅠ水平、PGR水平均逐渐降低,HP感染阳性率逐渐升高,其中Ⅳ期组胃炎患者的血清PGⅠ水平、PGR水平均明显低于Ⅲ期组、Ⅱ期组和Ⅰ期组,HP感染阳性率明显高于Ⅲ期组、Ⅱ期组和Ⅰ期组,Ⅲ期组胃炎患者的血清PGⅠ水平、PGR水平均明显低于Ⅱ期组和Ⅰ期组,HP感染阳性率明显高于Ⅱ期组和Ⅰ期组,Ⅱ期组胃炎患者的血清PGⅠ水平、PGR水平均明显低于Ⅰ期组,HP感染阳性率明显高于Ⅰ期组,组间比较差异均有统计学意义(P<0.05);经Pearson相关分析结果显示,PGⅠ、PGR与OLGIM分期均呈负相关(r=-0.607、-0.528,P<0.05),HP感染阳性率与OLGIM分期呈正相关(r=0.720,P<0.05)。结论 血清PGⅠ水平、PGR在Ⅳ期胃炎患者中的表达水平明显降低,HP感染阳性率有明显升高,血清PGⅠ水平、PGR及HP感染状况与OLGIM分期进展程度有明显相关性。 Objective To investigate the status of pepsinogen Ⅰ(PGⅠ), pepsinogen ratio(PGR), and Helicobacter pylori(HP) infection in patients with gastritis with different operative link for gastric intestinal metaplasia assessment(OLGIM) stages. Methods A total of 120 gastritis patients admitted to the Department of Gastroenterology, Xi’an High-tech Hospital from January 2019 to January 2021 were retrospectively selected and included in the observation group, and 110 healthy people who underwent physical examination in the hospital during the same period were selected as the control group. The serum PGⅠ level, PGR, and HP infection status of the two groups of subjects were compared,and the serum PGⅠ level, PGR, and HP infection status of patients with different OLGIM stages of gastritis were compared. Pearson correlation analysis method was used to analyze the correlation between serum PGⅠ level, PGR, HP infection positive rate and OLGIM staging. Results The serum PG Ⅰ level and PGR in the observation group were(74.89±3.71) ng/mL and 5.16±1.01, which were significantly lower than(95.40±5.85) ng/mL and 7.43±1.60 in control group(P<0.05). The positive rate of HP infection was 66.67%, which was significantly higher than 13.64% in the control group(P<0.05). The levels of serum PGⅠ and PGR in the four groups of patients with different OLGIM stage gradually decreased, and the positive rate of HP infection gradually increased;the serum PGⅠ level and PGR were significantly lower in stage Ⅳ group than stage Ⅲ, Ⅱ, Ⅰ groups, in stage Ⅲ group than stage Ⅱ, Ⅰ groups, and also in stageⅡ group than stage Ⅰ group;the positive rate of HP infection was significantly higher in stage Ⅳ group than stage Ⅲ,Ⅱ, Ⅰ groups, in stage Ⅲ group than stage Ⅱ, Ⅰ groups, and also in stage Ⅱ group than stage Ⅰ group;the differences were statistically significant(P<0.05). Pearson correlation analysis showed that PGⅠ, PGR were negatively correlated with OLGIM staging(r=-0.607,-0.528, P<0.05), and the positive rate of HP infection was positively correlated with OLGIM staging(r=0.720, P<0.05). Conclusion The levels of serum PGⅠ and PGR were significantly decreased in patients with stage Ⅳ gastritis, and the positive rate of HP infection was significantly increased. The levels of serum PGⅠ,PGR, and HP infection were significantly correlated with the progression of OLGIM staging.
